SIT program student evaluation forms may be requested by both departments, programs, and other academic units (referred to hereafter as departments) for classes which they offer and by individual instructors for their own classes. Forms to request SIT program student evaluation forms for both departments and individual instructors may be downloaded using the links below. Departments
Departments and programs may request that all courses for which they are responsible be evaluated. To request forms, a departmental representative should contact the Teaching and Learning Center at 413-9166 or via e-mail at teaching@uic.edu. Departments will then be sent a copy of informtation on their courses contained in the Timetable database and aksed to make any additions or deletions as well as confirm instructor names and enrollments.

Course evaluation packets will be sent to departments two (2) weekis prior to the scheduled end of classes for the Fall and Spring semester and one (1) week for Summer term. If course evaluations are needed earlier in the term, special arrangements can be made by contacting the Teaching and Learning Center at 413-9166.

The SIT Program form instrument is designed as a course rather than an instructor evaluation instrument. One set of evaluations will be sent for each course for which evaluations are requested. If a course is team-taught or there are discussion or lab sessions attached to a course for which student evaluation information is desired, please contact the Teaching and Learning Center to make arrangements. We can accomodate such circumstances if we know about them ahead of time. It is impossible to seperate the data once it is collected.

Individual Instructors
Participation in the SIT program is open to all UIC instructors. Instructors should check with their department or program office before contacting the SIT program to ensure that their department or program has not already registered their courses for the program. Participation in the SIT program does not exempt an instructor from participating in any existing department, program, school, or college student evaluation system.

Instructors must inform the Teaching and Learning Center of their desire to participate no later than three (3) weeks prior to the scheduled end of classes for the Fall and Spring semesters and two (2) weeks for Summer term. Requested forms will be delivered at least two (2) weeks before the scheduled end of classes for Fall and Spring semesters and one (1) week for Summer term. Please do not request forms from Undergraduate Student Government. All forms must be requested from the Teaching and Learning Center.

Instructors may also request forms by contacting the Teaching and Learning Center directly by phone at 413-9166 or by e-mail at teaching@uic.edu. Instructors are asked to provide information regarding the instructor’s name, department, course number(s), enrollments in each course for which evaluations are being requested, and each course's unique five-digit call number (from the Timetable). This information is vital to making sure that the data is analyzed and reports are distributed to faculty in a timely manner.

An e-mail confirmation will be sent to each instructor upon receipt of a completed request form.
