Instructions for Returning Completed Student Evaluation Forms

Our primary goal is to provide reliable and meaningful feedback on instruction in a timely and efficient manner. We also strive to ensure that all data will be accessible in the future. To facilitate these goals, we ask that participants in the Students for the Improvement of Teaching (SIT) Program follow these guidelines in returning completed student evaluation forms:

• Completed SIT Program student evaluation forms should be returned by a student to the drop box outside the Teaching and Learning Center in Douglas Hall. The drop box is located just outside Room 102, directly across the hall from the UIC Writing Center.

• Completed forms should be returned in the same envelope in which the blank forms were distributed. The label on the outside of the envelope provides vital information needed to process completed forms. If forms are returned in a different envelope, processing will be delayed and the chance of errors are greatly increased. Please take care to ensure that completed forms are returned in the envelope for the course for which they are intended to evaluate. We cannot separate completed forms for multiple courses returned in the same envelope.

• If there is an error on the label on the outside of the envelope, please make any necessary correction on the outside of the envelope. Please leave the label intact so we can make changes for the correct record in our database. Please include a means of contact on the outside of the envelope if you make corrections to the label. This will allow us to contact you if we have any questions.

• Please return completed forms directly to the Teaching and Learning Center. Please do not collect completed forms in departmental or program offices. This delays processing and reduces the credibility of the program. Departments and programs wishing to keep track of returned forms may request updates on the courses for which student evaluations have been returned from the Teaching and Learning Center.

• Completed student evaluation forms should be returned no later than one (1) week after the conclusion of final exams. Forms received later than that date will not be processed until after all processing is complete for forms received prior to that date.
