Transcripts

The official transcript is the University's statement of a student's academic record. The official transcript is printed on red security paper and is issued by the Office of Registration and Records. It lists all courses completed and grades earned in chronological order by term. Transcripts also contain notations of distinctions, transfer coursework summary, academic status, administrative actions and degrees received.

There is a $5 charge for each transcript.

Immediately after a term's final exams have ended, transcripts listing final grades may not be available for a period of up to two weeks. Transcripts listing final degree awarded may not be available for a period of four to six weeks following final exams of the final term in which the degree is earned. Transcripts will not be released if there is a hold on the student's record.

Transcripts may be requested in one of four ways:

  1. online using Student Self-Service options (students enrolled Fall 2004 and after)
  2. online using a system that does not require an Enterprise ID and password (students graduated prior to Fall 2004 or students who cannot recall their Enterprise ID and Password),
  3. in person, or
  4. in writing.

Ordering Transcripts Online

Students enrolled Fall 2004 and after

Request a transcript through Student Self-Service. You can also type https://apps.uillinois.edu in your web browser to enter Student Self Service. Select the Registration and Records menu, then Student Records and Transcripts, and finally select Official Transcripts.

Students graduated prior to Fall 2004 (or students who cannot recall an Enterprise ID and Password)

If you do not have an Enterprise ID and password, submit an online transcript request. The signature page must be faxed to complete your order.

The site will walk you through placing your order, including delivery options and fees. You can order as many transcripts as you like in a single session. A $2.25 processing fee will be charged per recipient. Order updates will be emailed to you. You can also check order status or history online.

Ordering Transcripts in Person

To order transcripts in person, visit the Office of Registration and Records, Transcript Window, located at 1200 West Harrison Street [directions and parking]. Office hours are Monday through Friday, 8:30 am to 5 pm.

Ordering Transcripts in Writing

Transcripts may also be requested in writing. The request should include:

  • Full name (please include maiden name)
  • University Identification Number (UIN) if you were enrolled in 1981 or later. You'll need your SSN and birthday to look up your UIN.
  • Social Security Number (SSN) is optional. If UIN unavailable, providing a SSN will greatly aid us in locating records.
  • Current Address
  • Date of birth
  • Dates of attendance
  • Student's college
  • UIC Degree(s)
  • Name(s) and address(es) to where transcript will be sent
  • Student's original signature (unsigned requests cannot be processed)
  • A check or money order payable to the University of Illinois at Chicago

Please note that transcripts are mailed first class standard via the United States Postal Service. If you require express mail service delivery, contact the Transcripts office at (312) 996-9575, Monday through Friday, 8:30 am to 5 pm central standard time, to make payment arrangements.
