Apply to UIC – Graduate Certificate Program
This page is designed to aid Graduate Certificate program applicants. We highly recommend you follow each step carefully. Mistakes can delay the processing of your application. Please keep this reference page open as you proceed through the application process.

1. Determine Application TypeRead through steps 1 through 3 in order to determine your application type. You will be prompted to begin the application in step 3. |
2. Check DeadlinesCheck the graduate application deadlines for the term to which you are applying and the admission requirements for the program of interest (choose from programs in the Nondegree and Campus Certificates box). Please note that admission requirements and documents vary for individual programs. |
3a. Domestic or InternationalBefore proceeding, determine if you are a Domestic or International Applicant. Domestic - an applicant who fits into any of these categories:
International - The university considers any student who currently holds a visa of any type or is seeking a visa to be international. Even if you do not require a student (F or J) visa to attend UIC, you are viewed as an international applicant. Most policies that apply to those seeking student visas will apply to holders of other types of visas. For the university's purposes, this is anyone who currently holds a visa of any type or who is in need of an I-20 or DS-2019. 3b. Apply online |
4. Create New AccountCreate a new account by clicking "First time user account creation." You will be asked to create a Login ID and six digit Personal Identification Number. Click the image on the right for a preview. |
5. Application Type: Campus Certificate or IBHE CertificateSelect your application type. Your options will differ depending on whether you intend to study for a Campus Certificate or IBHE.

6. Select your admission term and fill in your first and last name. |
7. ChecklistNext you will be brought to a Checklist page. Click on a section name to access that area of the web application. Fill out each section completely. When you have completed a section, a red checkmark will appear by its name.
International applicants should complete the additional section titled "International Information."

8. NameStart by filling out your name. This should be your full legal name as it would appear on all your other documents; do not use nicknames or abbreviations. Please use upper and lower case when entering information. For instance, “JOHN SMITH” should be entered as “John Smith.” Click the "Checklist" button to return to the Checklist menu. Click the "Continue" button to proceed to the next section, "Permanent Address Information." If at any time you wish to quit and resume applying later, click "Finish Later." |
9. Permanent Address InformationFill out your permanent address and telephone number. Do not abbreviate using periods, the pound sign (#) or commas. Here is an example of correct address entry: 123 South Monroe Street. This contact information will be added to your permanent record. After finishing this application, this information can only be changed by submitting a written request to the Office of Admissions. |
10. Mailing Address InformationEnter your mailing address. All correspondence from UIC will be sent to this address. Again, please do not abbreviate. |
11. Personal InformationCitizenship
Fill out the rest of your personal information. Be sure to verify your email address for accuracy, as it may be used to contact you throughout the application process. The same care should be applied when filling out your birth date, especially the year. If you are a resident of Illinois, check "yes" to the question "Are you an Illinois resident?" If you have questions regarding your residency status, refer to the University-Wide Student Programs Web site. |

14. Previous CollegeList all universities/colleges you have attended. Use the "Lookup College Code" link to search for your college/university. If your school does not appear to be listed in the lookup, first try searching with "unknown" for the city. If this too fails then type in the name of your institution. |
15. Test Scores - IBHE applicants onlySelf report your test scores here. Please have official reports sent from ETS. Our test code is 1851. |

18. Back to the ChecklistWhen all sections have been completed, a checkmark will be displayed next to each section's name (as indicated to the right). If you want to pay the non-refundable application fee with a credit card, do not press "Application is Complete" until you are ready to submit your credit card information. If you are paying by check or money order, or are ready to submit your credit card information, press "Application is Complete". |
19. Non-refundable Application Fee PaymentThis is where you determine the method of payment. Application fee waivers are only available for applicants who qualify. Paying Application Fee UIC Online applications require payment by credit card upon submission. No application fee is required for Summer Session Only. Paying Application Fee for Submitted Applications (Pay Later Option) Do not submit a second application to make a credit card payment. If you do not pay the nonrefundable application fee upon submission, you may pay later by credit card, check or money order. After submitting the application you will receive an acknowledgement letter from the University in 3 to 5 business days containing the information needed to pay by credit card. 21. Submit required credentialsSend transcripts, non-refundable application fee (unless you pay online), and Declaration and Certification of Finances (international applicants) to: Check your program listing for information regarding any additional credentials.
