Registration Fees

If you are unsure whether a discount may apply to your registration, contact UIC External Education.

 Registration fee includes:  

• Continental breakfast
• Lunch
• Beverage breaks
• All workshop materials

How to Register

There are four convenient ways to register for the 2011 UIC e-Government Workshop:
1. Online: Access the Online Registration System.
Online registrations are accepted 24 hours a day, 7 days a week.

2. Phone: Call UIC External Education at (312) 355-0423.
Business hours are Monday through Friday, 9 a.m. to 5 p.m. Central Time.

3. Fax: Download the Registration Form, and fax to (312) 413-9730.
Registration Forms may be sent to the attention of UIC External Education.

4. Postal Mail: Download the Registration Form, and mail to:

Payment Information

Acceptable forms of payment include:

• Credit card (Visa, MasterCard, Discover, American Express)
• Check
• Money order
• Purchase order (P.O.)

Checks and money orders should be made payable to the University of Illinois and can be mailed to:

Purchase orders can be faxed to (312) 413-9730, Attention: UIC e-Government Workshop.

Be sure to indicate the name(s) of the attendee(s) on the check, money order or purchase order.

Registrations are not complete until payment is received. Registrations accompanied by credit card payment guarantee your registration. If paying by check, money order or purchase order, payment must be received within 10 business days of your registration or by the start of the workshop, whichever comes first. If you anticipate any delays in submission of your payment, please contact UIC External Education. Failure to do so may result in a cancellation of your registration. You will receive written notification via e-mail if your registration is canceled due to lack of payment.

Registration Deadline and Cancellation Policy

The deadline to register for the workshop is Friday, May 6, 2011.
 
Cancellations must be made on or prior to May 2, 2011 in order to receive a refund less a $50 processing fee. To cancel, either access your registration via the link provided in your registration confirmation or send an e-mail to UIC External Education, including your confirmation number in the subject line.

Cancellations received after May 6, 2011 will not be refunded.
