Complete one UIC Experience Documentation Form for each activity and attach supporting
proof of engagement for each event. (Only document one activity per form. You will need to fill
out a total of 6 forms to earn the UIC Experience Certification for 2012-2013.)

Supporting proof can be items such as event ticket stub‚ event registration forms‚ a signed
faculty/staff letter‚ etc.
Bring documentation of your 6 experiences to the Campus Programs Department‚ Room 340‚
Student Center East‚ 750 South Halsted‚ to earn your certification for this school year by April
30‚ 2013.
