Survey Instructions

Thank you for choosing to provide your feedback on the Student Affairs’ Unit you have recently visited. Your feedback is important to us.

What should you expect?

This Online Student Satisfaction Survey is for students only. In order to ensure that only students complete the survey, you will be required to login to Bluestem using your NetID and password. This information will not be captured nor monitored in order to allow your feedback to be anonymous. You will receive an email based on your Bluestem login to thank you for participating in the survey. The survey is very short and should take less than five minutes for you to complete. If you provide your contact information in the survey, you will be contacted to discuss your feedback. Responses to feedback received will be place on the Survey Responses tab for students to view.

How to access the survey:

1. Click on the URL listed below. If clicking the link does not take you to the survey, you may need to copy and paste the URL onto your webpage.
2. Enter Netid and Password
3. On the survey entrance page, click the "Start Survey" button.
4. Answer all questions in order to arrive at the end of the survey.
