SLDVS

Nominations can be submitted by one's self, fellow students, staff, faculty, or volunteer coordinators.
A person can nominate as many UIC students as they like.
Only serious nominations will be taken into consideration.
Students must meet minimum GPA restrictions and be full time students during Spring 2012.

Nominations are collected through on on-line form and must include*:

• student's first and last name

• UIN
• UIC e-mail address
  
• text explaining/supporting the reason for the nomination
  

A nominated student receives an auto-generated email after a nomination requesting contact information. This is required for the printed invitation.  A nominated student must complete this portion to ensure consideration of nomination.
