Supplemental Application Instructions for the M.S. and Ph.D.
Degree Programs in Public Health Sciences

The following information supplements the instructions that are included with the enclosed graduate college application. Both sets of instructions should be read thoroughly.

Deadlines:

    January 1, 2005 - International Applicants
    February 1, 2005 - Domestic Applicants

Applicants are required to submit the following documents to the Office of Graduate Admissions (m/c 018), The University of Illinois-Chicago, Box 5220, Chicago, Illinois 60680-5220:

    1) A signed application form.

    2) Transcripts from all colleges and universities you have attended.

    3) An application fee of $40.00 for domestic applicants and $50.00 for International applicants.

    4) Graduate Record Examination (GRE) scores (Aptitude Test Only). An official test score report must be sent directly to UIC.

Applicants are required to submit the following documents to the School of Public Health (m/c 923), The University of Illinois-Chicago, 1603 West Taylor Street, Chicago, Illinois, 60612-4394, Attention: Admissions Officer

    5) A personal statement. The statement should include: a) why you wish to come to the School of Public Health; b) the area of concentration you have chosen and your reason(s) for the choice, c) research interests you may have, and a short statement regarding your career objectives and goals, and d) whether you would like to attend full-time or part-time.

    6) Three (3) letters of recommendation from either past or current instructors or individuals under whom you have worked, e.g., supervisor, director, head of the agency or department, etc.

Most students will be notified no later than June 1, 2004 regarding application decisions. Any questions regarding the admission process should be directed to the Schools Admissions Officers, Stephanie Taylor-Foster and Antonia Flynn:
