M.Ed. Application Instructions
Doctoral Application Instructions

Letter for Letters of Recommendation (M.Ed.)
This form should be used for the letters of recommendation which
will be submitted in support of your application for the M.Ed.
program in the College of Education at UIC. Please check the individual
program you are applying for because not all programs require
recommendations. You can enter your information on this form
on-line and then print it out.

Letter for the Statement of Goals/Purpose Statement (M.Ed.)
This form should be used with the essay/goals statement which
you will write for your M.Ed. application. Please check the individual
program you are applying for because not all programs require
an essay.
Work Experience
Form
This form should be used when applying for the M.Ed. in Literacy,
Language and Culture. If you need more than one page, just print
multiple pages and staple them together for submission. You can
fill the form in online and print it or you can print it and fill
the form in by hand. A current resume may also be submitted in
place of these forms.
Early
Childhood Education admissions Questionnaire and Essays
This questionnaire should be used to complete all the requirements
for application to the M.Ed. program in Early Childhood Education.
You can enter your information on this form on-line and then
print it out.
College of Education
Doctoral Application
This application should be used in conjunction with the official
UIC Graduate Application. Both must be submitted to be considered
for the Doctoral program. You may complete this form on-line,
print it and submit it to the College of Education with the other
appropriate credentials.
Cover Letter for
Letters of Recommendation (all Doctoral programs)
This form should be used for the letters of recommendation which
will be submitted in support of your application for the Doctoral
programs in the College of Education. You can type in your pertinent
information, print the cover sheets and provide them to your letter
writers.

Letter for the Statement of Goals (All Doctoral programs)
This form should be used with the goals statement which you will
write for your Doctoral program in the College of Education. You
can type in your pertinent information, print this form and submit
it attached to your statement of goals.
Application for Admission
to the College of Education (Bachelors Degree only)
This form is used to apply for admission to the BA in Elementary
Education. If you are applying to the College of Education and
are already a UIC student, this is the only application form you
will need to complete. If you are transferring to the College
of Education from another school (and are eligible to begin in
the College of Education instead of Pre-Elementary Education in
Liberal Arts and Sciences) you must complete this application
and the general Undergraduate application for admission. If you
already have your Undergraduate degree, you should be applying
for the M.Ed.
Checklist for Admission
to the College of Education (Bachelors Degree only)
This form can be used to keep a record of all the materials you
must submit for admission to the Undergraduate College of Education
program in Elementary Education.
Service
Learning Hours Form (Bachelors Degree Only)
This form should be completed and submitted with your application
for the BA in Elementary Education.
BA Writing Sample
(Bachelors Degree only)
This is the information you will need to complete your writing
sample for you application to the BA in Elementary Education.
Cover Letter for Letters
of Recommendation (BA in Elementary Education)
This form should be used for the letters of recommendation which
will be submitted in support of your application for the BA program
in Elementary Education in the College of Education at UIC.
