Honors Activity Agreement Form
Complete and submit by 4pm Friday, of the third week of each semester.

Complete and submit by 4pm Friday of exam week.
Information Update form
This form is used to assist the college in maintaining the most up-to-date information on our students. While areas such as name, address, major and phone can be updated quickly, two requests require additional procedures.
A request for a new fellow can be accommodated as space allows. Each fellow is assigned to a maximum of 6 students at any one time. When a request would pass this limit for a fellow-that fellow is contacted to ask if they would be willing to take on an additional student. Regardless of whether or not the request can be filled, each student will be contacted and notified of the status of their request.
The last section of the form allows for a student to request to be added to the following courses: HON 202-Honors College Tutoring, HON 222-Honors Activity and HON 225, Honors Research, all require a few weeks for processing as these requests must be routed through the Office of Admissions and Records.
Petition Form
For “special circumstances” such as: request to be reinstated, withdraw from the honors college, waiving the Honors Activity requirement for a term, permission to complete more than one activity for a term, to remove any deferred grades form HON 222, 202, 225 from previous semester.
Students must complete the form by including an explanation of their reason for filing the
petition. The completed form should be submitted to the front desk for review. Each submitted petition is reviewed by Dean Janet Madia and students are informed of the decision within one week of submitting their petition. If further discussion is required, students will be asked to come in to meet with Dean Madia for a final decision to be made.
Honors College Graduation Declaration Form
Tell us about your graduation plans. Submit this form 45 days before you graduate (usually late March). It does not replace the graduation declaration requirements of your academic College.
Alumni Prospectus
The Honors College is eager to learn about your post-graduation plans. Please let us know, to the best of your knowledge, what your plans are. Please submit this information to us by Friday of the last week of classes.
Honors College Facility Request Form
Used by honors college students for reserving rooms within the college. Students must return the completed form the reception desk at least one week prior to day of event. Final confirmation will be sent to the e-mail address provided on form.
Honors College Capstone Proposal and Registration Form
Students must fill out their form when working on their Capstone project in order to receive credit.
Honors College Tutoring Program
Honors College Tutoring is a volunteer tutoring service that takes place in the Honors College Tutoring Center, located on the second floor of Burnham Hall in Room 220. Honors College Tutoring is a walk-in service to serve all UIC students throughout the campus with one-on-one academic help in a particular class.
