Patients who are at risk for repeat hospital admissions due to chronic conditions, often require close home monitoring to prevent any exacerbation of their condition. The Home Health Monitor Telemanagement Service established at UICMC performs ongoing disease telemanagement utlizing the Home Health Monitor (HHM) and the objective physiologic data it provides. This telemanagement includes medication titration, patient supervision and education. Who Should Be Monitored?
Patients with:
  • Heart failure (CHF)
  • Hypertension (High blood pressure)
  • Pulmonary hypertension
  • Diabetes
  • Cardiac transplant patients
  • Atrial fibrillation (Anticoagulation)
  • Asthma
  • Post-surgical recovery
