Home Health Monitor Service Compared to Conventional Home Health Care for patients with Congestive Heart Failure:

Patients will be randomized to receive either conventional home health care or use the HHM for 2 months. All follow-up data will be collected for 12 months after enrollment

Objective: to determine which method of CHF follow-up care will be most effective at:

  • Decreasing hospital re-admissions for CHF
  • Decreasing CHF healthcare cost
  • Improving quality of life

Inclusion criteria: (Any ONE of the following)

  • Documented evidence of ventricular dysfunction by echocardiograph
  • Clinical signs/symptoms of CHF that improve with appropriate treatment
  • Documented NYHA functional class III or IV
  • History of least 2 CHF admissions within the past 12 months
  • Have an operating telephone line

Exclusion criteria: (Any ONE of the following)

  • Unstable angina
  • End-stage CHF requiring IV infusions of inotropic or diuretic agents
  • Renal failure/dialysis
  • Planned discharge to a long-term care facility
  • Severe dementia
  • Anticipated survival of less than 6 months
  • Home health nursing care specifically for CHF (applies to the monitor group only)

The Effects of Telemanagement with Transtelephonic Monitoring of Hypertension in African Americans

Objectives:

  • To determine if African Americans with HTN will be willing to maintain a high level of monitoring, using the Home Health Monitoring System for three months
  • To determine the effects of a telemanagement system consisting of physician supervised evaluation/titration/changes of medications and nurse directed patient education
  • To improve hypertension control

Inclusion criteria: (all of the following)

  • African American race - greater than 18 but less than 70 years of age
  • Diagnosis of HTN (>140mmHg systolic and/or 90mmHg diastolic)
  • Medically stable (abscence of co-morbid conditions)
  • Cognitively capable of consenting to study participation
  • Have an operating phone line
