Who Should Be Monitored?
- Patients at risk for repeat hospital admissions related to chronic
conditions
- Patients who can benefit from closer monitoring of vital signs
- Patients who could be candidates for early hospital discharge if
certain physiologic variables and lab values could be monitored and safely managed from
home
- Patients in need of close titration of medications such as ACE
inhibitors, beta blockers, calcium channel blockers, diuretics, anticoagulants, insulin,
home oxygen monitoring and others.
What can be monitored:
- Weight (heart failure and hypertension)
- Blood Pressure (heart failure and hypertension)
- Heart Rate (heart failure, hypertension, and atrial
fibrillation)
- Glucose (diabetes)
- Oxygen Saturation (heart failure,
hypertension, chronic obstructive pulmonary disease)
- INR (atrial fibrillation, coagulation disorders, and
articifcial heart valves)*
- Spirometry (asthma)*
- Thermometry (post-surgical, oncology patients)*
*These functionalities are in development and are not available at

The Home Health Telemanagement Service includes:
- Installation, maintenance, and user training of the Home Health
Monitor (HHM) in the patient's home
- Initial assessment with HHM staff and referring physician to
determine care plan and alarm parameters
- Initial and continued patient/family disease management and education
by the medical staff
- Daily data transmission to a central computer server with alarm
capability
- Availability of 24 hour 'as needed' transmissions with medical staff
and physician intervention
- Customized management of all data with scheduled reporting to
referring physicians
Anticipated Benefits of the HHM Telemanagement Service include:
- Reduced risks associated with shorter lengths of hospitalization
- Reduced frequency of recurrent ER visits and hospitalizations
- Promotion of patient compliance with disease specific treatments to
improve patient outcomes
- Improvement in patient perceived health related quality of life
