Who Should Be Monitored?

  • Patients at risk for repeat hospital admissions related to chronic conditions
  • Patients who can benefit from closer monitoring of vital signs
  • Patients who could be candidates for early hospital discharge if certain physiologic variables and lab values could be monitored and safely managed from home
  • Patients in need of close titration of medications such as ACE inhibitors, beta blockers, calcium channel blockers, diuretics, anticoagulants, insulin, home oxygen monitoring and others.

What can be monitored:

  • Weight (heart failure and hypertension)
  • Blood Pressure (heart failure and hypertension)
  • Heart Rate (heart failure, hypertension, and atrial fibrillation)
  • Glucose (diabetes)
  • Oxygen Saturation (heart failure, hypertension, chronic obstructive pulmonary disease)

  • Spirometry (asthma)*
  • Thermometry (post-surgical, oncology patients)*

*These functionalities are in development and are not available at this time.
