Q - What is CampusCare?

A -CampusCare is a University-sponsored supplemental health benefit program offered to registered undergraduate and graduate students and their eligible dependents. All newly enrolled students who are assessed the Student Health Insurance Fee along with their tuition are automatically enrolled. There is no enrollment form to complete.

CampusCare is designed to provide protection against accidents and unexpected illnesses and covers health services such as inpatient and outpatient hospitalization, prescription drugs, physical therapy, home health care, mental health/substance abuse services, and emergency services.

Q - Is CampusCare coverage mandatory?

A - All students enrolled at the University are required to have supplemental health benefits coverage. If you can show proof of alternate comparable health insurance coverage you may “opt out” of the CampusCare program by submitting an on-line "Waiver Form" during the enrollment period at the beginning of the term.

Q - How does CampusCare work?

A - All eligible newly enrolled students are automatically enrolled in CampusCare. There are no enrollment forms to complete. Members have access to a range of health benefits and services with no claim forms to complete. Your CampusCare Health Center physician will provide and coordinate all your medical care needs. You can access your covered benefits by making an appointment with a physician or nurse practitioner at the UIC Department of Family Medicine. Call (312) 996.2901 to schedule an appointment.

As a member of CampusCare, you also have access to a network of dental, vision, and hearing providers who offer services on a reduced fee-for-service basis. There is also a discount prescription drug program as well as a life and accidental death and dismemberment benefit. See “Benefits" section for information and how to access these additional benefits.

Q - If I am enrolled in CampusCare can I go to any doctor I want?

A - NO! You must see a CampusCare Physician when you need medical attention. The physicians and nurse practitioners of the UIC Department of Family Medicine are the primary care providers for members at the Chicago campus. Call (312) 996.2901 to schedule an appointment.

If you are experiencing a life-threatening medical emergency, please call 911 or go to the nearest emergency room for treatment. Contact your CampusCare Health Center Physician at the number listed on your Identification Card and/or CampusCare at (312-996-4915) as soon as possible or within 48 hours to inform a representative of your medical situation and the care you have sought.

If you need a service not offered by a CampusCare Physician or facility, your CampusCare Physician will provide you with a referral and direct you to a specific provider.

Q - Why should I consider the CampusCare Program?

A - Good health care is essential for your academic success. When you need health care coverage, you want it to be dependable, efficient, effective, convenient and affordable so you can get back to studying and on with your life. A quality health benefits program is an important adjunct when seeking health care. The University of Illinois recognizes the importance of health benefit coverage and therefore offers the CampusCare program to all registered students.

CampusCare costs less then $3.00 a day and provides over 750 accessible, convenient and highly qualified doctors on campus. CampusCare has no deductible and provides several healthcare services at 100% coverage. Please visit the “Benefits” section to view the “Benefits at a Glance.”

Q - How do I obtain my CampusCare member ID card?

A - You can print your Member Identification Card off this website. No logon or password is necessary. Just go to “ID Cards” on the upper menu bar to find the student or dependent member ID card. ID cards are not mailed to members.

Q - I want to access the dental benefit. Do I use my CampusCare card?

A - NO. If you want to access the dental, vision, hearing, or discount pharmacy benefits offered to CampusCare members through United Health Programs you will also need to print the UHP Member ID card. Click on the "Benefits" section and scroll down to "Additional Benefits" and click on the "Dental" link. On the upper right corner, click the "Login Link" and complete the information required.

Q - Where can I find the forms I need?

A - All CampusCare forms are submitted electronically and are available in the "Forms" section.

Q - Where can I find more information about covered benefits?

A - Visit the "Benefits" section for a summary of benefits and the "Certificate of Coverage." This page also has information about the additional benefits available to members through United Health Programs and Reliance Standard Life Insurance Company.

Q - Is there coverage for maternity care?

A -CampusCare covers services related to pregnancy, including delivery, if provided or authorized by a CampusCare Health Center physician. If you have a question about covered services, contact a CampusCare Student Services Representative. If you are pregnant or expect to become pregnant, make an appointment to see your CampusCare Health Center physician to plan for your health care needs.

Q - Are occupational therapy services covered?

A - Occupational therapy is a covered benefit when medically necessary and authorized by a CampusCare Health Center Physician. You need to obtain a referral from your CampusCare physician in order to receive covered OT benefits under the program.

Q - Is there a dental or vision benefit?

A - Yes. CampusCare offers reduced fee-for-service on dental and vision services through United Health Programs. You can also take advantage of savings on hearing services and instruments as well as a discount prescription drug program. For instructions on how to access these benefits, visit the "Benefits" section and scroll down to the "Additional Benefits" link. Click on the "Vision" or "Dental" link(s). On the upper right corner, click on the "Provider Locator" link and follow the steps.

Q - Where can I find a list of providers participating in the dental, vision, hearing, or discount drug program offered through United Health Programs?

Q - Is there a life insurance benefit?

A - Yes. Life and Accidental Death and Dismemberment benefits are provided by the Reliance Standard Life Insurance Company to CampusCare members. Links to information and a beneficiary form are available on the "Benefits" section.

Q - Can I add a family member to my coverage?

A - You can add a spouse and/or children during the enrollment period at the beginning of each term by submitting the Dependent Enrollment Form. See “About CampusCare” for dependent eligibility information and the enrollment deadline date.

Q - I’m getting married and want to add my spouse. What should I do?

A - Members may add a new spouse and/or any eligible children of a new spouse within 31 days of marriage. A copy of the marriage certificate is required. Contact a Student Services Representative at 312.996.4915 option 3 for assistance with adding this type of coverage.

Q - I’m having a baby. How do I add him/her to my coverage?

A - Members must add newborn coverage within 31 days following birth. A copy of the birth certificate is required. Contact a Student Services Representative for assistance.

Q - I’m adopting a child and want to add her to my coverage. What should I do?

A - Members may add coverage for a child pending finalization of a legal adoption or a newly adopted child within 31 days of filing of the legal documents or of the legal adoption. Contact a Student Services Representative at 312.996.4915 option 3 for assistance.

Q - Is there an extra charge for dependent coverage?

A - Yes. There is an additional premium charge for a spouse and for children. See “About CampusCare” for current premium rates.

Q - How do I cancel my dependents’ coverage?

A - Dependent coverage may be cancelled during the enrollment period at the beginning of each term by submitting the "Dependent Disenrollment Form" by the enrollment deadline. See "About CampusCare for coverage dates and deadlines.

Q - Do I need to enroll my dependents each term?

A - Generally, no. When you add a spouse or children to your coverage they remain enrolled until your coverage terminates, such as upon graduation, or until you submit a "Dependent Disenrollment Form." You will automatically be billed for dependent premiums each term. There are, however, 2 exceptions:

  1. Summer Continuation Coverage. If you purchase summer continuation coverage for a summer term in which you are not enrolled you MUST also enroll your dependents by submitting a "Dependent Enrollment Form" along with your "Summer Continuation Enrollment Form." See “About CampusCare” for summer coverage information.

  2. Late Registrants. If you register late – after the 10th day of class for the fall or spring term or after the 5th day for the summer term – and have not waived out of the program, you will be automatically enrolled and assessed the full Student Health Insurance Fee. Dependents, however, are NOT automatically enrolled when you register late. If you want dependent coverage, you must submit the "Dependent Enrollment Form" before the enrollment deadline date. See “About CampusCare” for information for late registrants.

Q - I already have health insurance coverage and don’t need CampusCare. What should I do?

A - If you can show proof of alternate comparable health insurance coverage you can “opt out” of the program by submitting a "Waiver Form" during the enrollment period at the beginning of the term. See "About CampusCare for coverage dates and deadlines.

Q - Do I need to waive coverage every semester?

A - No. Students with approved waivers do NOT need to submit a "Waiver Form" each term. You will remain out of the program until you submit a Reinstatement Application Form during the enrollment period at the beginning of term in which you wish to reinstate coverage.

Q - I have an approved waiver. When can I expect to see a credit on my account?

A - It generally takes a week to 10 days for the credit to appear on your Student Account Summary Statement or eBill. You can access your account through the Student Financial Services www.sfs.uic.edu

Q - I can’t remember if I waived coverage. How can I check my status?

A - You can check your status easily by looking at your Student Account Summary Statement or eBill. If you were charged the Student Health Insurance Fee then you have not waived out of CampusCare. You can access your account through the Student Financial Services www.sfs.uic.edu. If you have a question after checking your account, contact a CampusCare Student Services Representative at 312.996.4915 option 3.

Q - I waived CampusCare. If I lose my health insurance coverage can I get CampusCare coverage?

A - Yes. You may reapply for coverage by submitting the "Reinstatement Application Form" during the enrollment period at the beginning of the term. You can also reinstate dependent coverage by submitting the "Dependent Enrollment Form." If you lose health insurance coverage during the middle of a term, contact a Student Service Representative at 312.996.4915 option 3.

Q - I’m covered under my parents’ health insurance plan but that’s going to end soon. What should I do?

A - If you are covered under your parents’ plan, make sure you know when the coverage ends! Plan to enroll in CampusCare during the enrollment period at the beginning of each term to ensure continuous health insurance coverage.

Q - I work at the University and plan on taking a class at UIC. I qualify for a waiver of my tuition. Does the tuition waiver also waive me out of CampusCare?

A - NO. The wavier of tuition and certain fees for eligible students employed by the University does NOT include the Student Health Insurance Fee. Some employees are not eligible for coverage under the Employee Benefit Program and may want or need coverage under the CampusCareprogram.

Regardless of a person’s employment status at the University, a newly enrolled student who is assessed the Student Health Insurance Fee as part of their tuition is automatically enrolled in CampusCare.

If you are a student who has health insurance coverage through your employment status with the University you can "opt out" CampusCare coverage by submitting the "Waiver Form" during the enrollment period at the beginning of the term for which you are enrolled.

Q - How much does CampusCare coverage cost?

A - The current premium for a single student is $401.00 for Fall, $401.00 for Spring, and $264.00 for the Summer term. The Student Health Insurance Fee premium is billed by the University along with your tuition and other fees. The additional cost for including a spouse and/or children can be found on this site under “About CampusCare.”

Q - Will my financial aid cover the premium cost for CampusCare?

A - Generally, yes. When granting financial awards, the University of Illinois at Chicago considers many expenses including tuition, books, room and board, and various fees such as the Student Health Insurance Fee, which is the premium charged for participation in CampusCare.

Please note, however, that some external agencies may stipulate that their financial assistance be used only for certain expenses, such as tuition. Contact your assigned Financial Aid Administrator in the Office of Student Financial Aid with questions regarding your financial aid package. The general phone number is 312-996-3126.

Q - When does coverage begin and end?

A - Coverage begins at 12:01 a.m. on the first day of class of the academic session and ends at 12:01 a.m. on the first day of class of the subsequent academic session, semester, or summer session. See “About CampusCare” for specific coverage dates for each academic term.

Q - Can I continue my coverage during the summer break?

A - Yes. If you are enrolled in CampusCare for the Spring term you may purchase coverage for the following summer break even if you do not enroll in summer classes. Submit the on-line "Summer Continuation Enrollment Form" by the published deadline and pay the summer CampusCare Fee, which is billed to your student account. Dependents are NOT automatically enrolled for coverage. If you want dependent coverage over the summer break, you must ALSO submit an on-line "Dependent Enrollment Form".

Students not enrolled in CampusCare for the Spring term are NOT eligible to purchase summer continuation coverage for the summer break.

Q - Can I extend my CampusCare coverage after I graduate?

A - No. CampusCare does not provide coverage after graduation. CampusCare is only available to students who are enrolled, pay tuition, and pay the Student Health Insurance Fee. However, visit the "Transition Benefits" section and click on College Community Insurance or any other company that offers individual and family health coverage to students who graduate.

Q - What should I do if I need to see a doctor?

A - Call (312) 996.2901 to schedule an appointment with a CampusCare physician or nurse practitioner at the Family Medicine Department at the UIC Outpatient Care Center or the Family Medicine Center in Univeristy Village. The location and hours of operation are listed below.

A CampusCare Health Center Physician will coordinate your medical care and provide authorizations for covered services to ensure your health care needs and subsequent claims are handled quickly and accurately. Your CampusCare physician must preauthorize all referrals.

Q - Do I need a referral to see a specialist?

A - Yes. Your CampusCare Health Center Physician will coordinate all of your care and can provide a referral to a specialist if necessary.

Q - What do I do if I have a life-threatening emergency?

A - In the event of a life-threatening emergency, call your local Emergency Medical Services by dialing 911 or go to the nearest hospital. Within 48 hours of receiving emergency treatment, or by the next business day, you must contact CampusCare at (312) 996-4915 for notification and authorization. CampusCare provides 100% coverage for life-threatening emergencies. However, if CampusCare determines that for any reason the services were not medically necessary or that the event was not a life-threatening emergency, coverage and payment will be denied.

Q - Where should I get my outpatient prescription filled?

A - The University of Illinois Medical Center Pharmacy is the preferred pharmacy provider. Students should have their prescriptions filled at the following pharmacy locations:

  • Students using UIC specialty clinics may continue to use affiliated pharmacies (i.e. DERM)
  • Saturday coverage for CampusCare patients at Wood Street Pharmacy will be discontinued effective July 2006

Present your CampusCare membership ID card and pay only a $10.00 co-payment for covered generic drugs a $20.00 co-payment for covered branded drugs or a $40.00 co-payment for Non-formulary drugs. Non-formulary Drugs covered with a $40 co-payment

If you are unable to access the Medical Center Pharmacy, you may take advantage of the Discount Prescription Drug Program, an additional benefit available to students participating in CampusCare.

Q - Can I get my prescription filled for more than just a few doses of medication?

A - If you are taking maintenance medications, you can receive up to a 60-day supply of medication from the University of Illinois Medical Center Pharmacy. Your physician must write a script for a 60-day supply and you will be asked to pay a double copayment.

Please note that the Medical Center pharmacy will only dispense a 60-day supply of medication if the 60-day period falls within the CampusCare benefit coverage dates.

Q - Are there any deductibles or copayments with CampusCare?

A - There are no deductibles. However, students are responsible for a copayment for all physician office visits and a copayment for covered medications of $10.00 for generic mediciations $20.00 for brand medications and $40.00 for Non-formulary mediciations. Some ancillary benefits such as vision and dental services require payments at the discounted price.

Q - Are there any claim forms to complete?

A - No. There are no claims forms to complete. However, if you take advantage of the Discount Prescription Drug program, you will need to submit a request for reimbursement. Instructions on how to submit a request for reimbursement is available in “About CampusCare.”

Q - I received a bill for services I thought were covered under CampusCare. What should I do?

A - If you receive a bill for any type of service that you believe is a covered benefit send the bill to:

Q - How am I reimbursed for covered outpatient prescription drugs obtained through the Discount Prescription Drug Program?

A - Present your discount prescription program member ID card at any participating retail pharmacy, or use the discount prescription mail order program. Then mail the following items to CampusCare at the address below:

  • Your full name, mailing address, and UIN (University Identification Number)
  • Original payment/paid receipt (The University will NOT accept copies!)
  • Prescription Identification Sheet (the pharmacy-generated sheet attached to dispensed medications)

You will be reimbursed by mail for the cost of your covered prescription drugs less a $10.00 co-payment for covered generic drugs or a $20.00 co-payment for covered branded drugs and $40.00 co-payment for Non-formulary drugs. Q - I’m going to take a semester off. Can I continue my CampusCare coverage while I’m away?

A - No. CampusCare does not offer coverage to individuals who take a “leave of absence” from school. If you are enrolled in CampusCare and plan to take a semester off, you may wish to contact College Community Insurance to purchase health coverage for the time you are away from school.

Q - If I participate in a UIC Study Abroad Program, can I keep my CampusCare coverage?

A - Yes. CampusCare, which only covers health services provided in the United States, is available to students participating in recognized UIC Study Abroad Programs. In addition to the required international health insurance coverage, you may continue or reinstate CampusCare coverage for your term abroad to provide domestic health coverage in the event you need it.

If you enroll in a recognized UIC Study Abroad program session and have not previously waived CampusCare, your coverage will continue (along with any enrolled dependents) during your registered term abroad and you will be assessed the Student Health Insurance Fee(s). You may also enroll or dis-enroll dependents by submitting the appropriate forms during the enrollment period at the beginning of term during which you will be abroad.

You are not required to continue domestic coverage while abroad and you can waive out of the program for your term abroad by submitting a Waiver Form during the enrollment period at the beginning of the term.

Students studying abroad are required to purchase international health insurance coverage specifically designed to cover services provided outside the United States. Questions regarding international health insurance coverage may be directed to the UIC Study Abroad Office at (312) 413-7662.
