Dependent Enrollment Form

The dependent enrollment form allows enrollment of a student's eligible spouse and/or children.

 

The Dependent Enrollment Form is NOT for Domestic Partner enrollment into the CampusCare program. Please contact the Gender and Sexuality Center or the Office for Access and Equity at 312-413-8619 for inquiries regarding domestic partnerships.
