Student/MemberTo be eligible to enroll in CampusCare , an individual must be registered student of UIC. A student is eligible to enroll in CampusCare for the summer Term if he or she was enrolled during the previous spring Term and pays the premium. A student employed at UIC is eligible to enroll in CampusCare only if he or she is ineligible for any State of Illinois insurance benefits.
Dependent CoverageThe CampusCare program is available to qualified family members such a spouse, civil union and unmarried children. Students enrolled in CampusCare may enroll eligible dependents by completing the online Dependent Enrollment Form prior to the enrollment change period. Eligible dependents are defined as:
Waiving CoverageStudents who have comparable health insurance coverage, for example through a legal guardian or an employer, may waive out of the CampusCare program by submitting the online Waiver Form before the enrollment change deadline. Once a waiver has approved the waiver on your student account forever, a waiver remains in effect until a Reinstatement Application Form is submitted and approved. Students with approved waivers DO NOT need to submit a waiver each semester. Please note the following:
Reinstatement of BenefitsEnrolled students who have previously waived out of the CampusCare program may reapply for coverage by submitting the online Reinstatement Application Form before the enrollment change deadline. Students participating in CampusCare may also reinstate eligible dependents by submitting the Dependent Enrollment Form.
Students who waived their CampusCare coverage and request reinstatement within the enrollment change deadline will be effective the first day of the term. Students who request reinstatement after the enrollment change deadline will become eligible for CampusCare benefits the date the request is received in the CampusCare Administrative office. No adjustment in the premium will be made.
Summer CoverageStudents registered for classes for the summer semester will be automatically enrolled in CampusCare.
Students also have an opportunity to waive out of the program, reinstate benefits, enroll dependents or disenroll dependents by submitting the appropriate form before the enrollment change deadline.
Summer Coverage When Not Registered for Classes: Students enrolled in CampusCare for the spring semester who do not register for the summer semester may purchase summer coverage by submitting the online Summer Continuation Enrollment Form before the enrollment change deadline and paying the CampusCare fee which is billed to the student’s account. Dependents are NOT automatically enrolled. Students who wish to enroll dependents for summer continuation coverage must ALSO submit the Dependent Enrollment Form.
Students not enrolled in CampusCare for the spring semester who do not register for the summer semester are not eligible to purchase summer coverage.
