1. Can a CampusCare member add a family member to their coverage?
A spouse, domestic partner and/or child(ren) can be added during the enrollment period at the beginning of each term by submitting the Dependent Enrollment Form before the designated deadline. Eligible dependents are defined in the Certificate of Coverage:
- Spouse (marriage license must be provided)
- Dependent (Child(ren) - Birth certificate(s) must be provided.)
- Unmarried child under the age of nineteen (19), including a natural or legally adopted child as well as a child for whom the Member or his/her spouse is the legal guardian (birth certificate, adoption or legal guardianship papers must be provided)
- Unmarried child under the age of twenty-three (23) if a full time dependent student at an accredited institution of higher learning, who is not self-supporting (proof of current full time registration must be provided for each Term of enrollment)
- Domestic Partner:
- Shall mean an unrelated partner, same sex individual who reside in the same household and have a financial and emotional interdependence, consistent with that of a married couple for a period of not less than one year and continue to maintain such arrangement.
- To qualify, a completed University of Illinois Affidavit of Domestic Partnership form, or its equivalent from another governmental entity, must be submitted and approved by the Gender and Sexuality Center or the Office for Access and Equity.
- If there is a change in status as domestic partners, the student agrees to notify the Gender and Sexuality Center or the Office for Access and Equity within thirty (30) days of such change by filing a statement of Termination of Domestic Partnership, which will make the domestic partner no longer eligible for coverage.

2. What is the process to add a new spouse to my CampusCare coverage?
Members may add a spouse and/or eligible dependents of a spouse within 31 days of marriage. A copy of the marriage certificate and copies of all applicable birth certificate(s) are required. Contact a Student Services Representative at 312.996.4915 option 3 for assistance when adding this type of coverage.
3. How do I add a new baby to my CampusCare coverage?
Members must add a newborn to CampusCare coverage within 31 days following birth. A copy of the birth certificate is required. Contact a Student Services Representative at 312.996.4915 option 3 for assistance when adding this type of coverage.
4. Is there an extra charge for the dependent coverage?
Yes. There are additional premium charges for a spouse, domestic partner and for dependents. See Fees and Dates for current premium rates.
5. How do I cancel my dependents' coverage?
Dependent coverage may be cancelled during the enrollment period at the beginning of each term by submitting the online Dependent Disenrollment Form before the designated deadline. See Fees and Dates section for current deadlines and premium rates for each term.
6. Are dependents automatically re-enrolled each term?
Yes. When a spouse, domestic partner or dependents are added to CampusCare coverage, they remain enrolled until CampusCare coverage terminates, such as upon graduation, or a Dependent Disenrollment Form is submitted as long as premiums are paid in full. You will automatically be billed for dependent premiums each term. The 2 exceptions are as follows:
- Summer Continuation Coverage. If you purchase summer continuation coverage for the summer term in which you are not registered for classes you MUST also enroll your dependents by submitting a Dependent Enrollment Form along with your Summer Continuation Enrollment Form.
- Late Registrants. If you register late – after the 10th day of class for the fall or spring term or after the 5th day for the summer term – and have not waived out of the program, you will be automatically enrolled and assessed the full CampusCare Student Insurance Fee. Dependents, however, are NOT automatically enrolled when you register late. If you want dependent coverage, you must submit the Dependent Enrollment Form before the enrollment deadline date.
