1. Are there any deductibles or co-payments associated with the CampusCare program?There are no deductibles. However, students are responsible for copayments for physician office visits and covered prescription medications. Some Ancillary benefits, such as discounted vision and dental services, require payments at the discounted price only.

2. Are there any claim forms to complete?No. There are no claim forms to complete. Present your Discount Prescription Drug Program Member ID Card at any participating retail pharmacy, or use the discount prescription mail order program. Then mail the following items to CampusCare at the address below:
