To qualify, a completed University of Illinois Affidavit of Domestic Partnership form, or its equivalent from another governmental entity, must be submitted and approved by the Gender and Sexuality Center or the Office for Access and Equity. If there is a change in status as domestic partners, the student agrees to notify the Gender and Sexuality Center or the Office for Access and Equity within thirty (30) days of such change by filing a statement of Termination of Domestic Partnership, which will make the domestic partner no longer eligible for coverage. Please direct all inquiries regarding domestic partnerships to the Gender and Sexuality Center or the Office for Access and Equity at 312-413-8619. Waiving Coverage Students who have comparable health insurance coverage, for example through a legal guardian or an employer, may waive out of the CampusCare program by submitting the online Waiver Form before the enrollment change deadline. Once approved, a waiver remains in effect until a Reinstatement Application Form is submitted and approved. Students with approved waivers DO NOT need to submit a waiver each semester. Please note the following: Waiver Form must be submitted online before the enrollment change deadline. Information about the comparable health insurance coverage is required: Name of the insurance company Member ID number Group Number Insurance company Customer Service Phone Number CampusCare will verify that the coverage is sufficient. CampusCare will provide e-mail notification of acceptance or denial of waiver request. Waivers submitted after the deadline will be denied. Waivers denied due to inadequate health insurance coverage will be denied. Reinstatement of Benefits Students who have previously waived out of the CampusCare program may reapply for coverage by submitting the online Reinstatement Application Form before the enrollment change deadline. Students participating in CampusCare may also reinstate eligible dependents by submitting the Dependent Enrollment Form. Students who waived their CampusCare coverage and request reinstatement within the enrollment change deadline will be effective the first day of the term. Students who request reinstatement after the enrollment change deadline will become eligible for CampusCare benefits the date the request is received in the CampusCare Administrative office. No adjustment in the premium will be made. Summer Coverage Students registered for classes for the summer semester who were registered for the previous spring semester and paid the CampusCare fee will be automatically enrolled in CampusCare (including enrolled dependents). Students also have an opportunity to waive out of the program, reinstate benefits, enroll dependents or disenroll dependents by submitting the appropriate form before the enrollment change deadline. Students enrolled in CampusCare for the Fall semester but who take a leave from school for the following Spring semester (did not enroll in class and paid no tuition or fees) are not eligible for summer coverage unless they register for the summer semester and pay the Student Health Insurance Fee. Summer Coverage When Not Registered for Classes: Students enrolled in CampusCare for the spring semester who do not register for the summer semester may purchase summer coverage by submitting the online Summer Continuation Enrollment Form before the enrollment change deadline and paying the CampusCare fee which is billed to the student’s account. Dependents are NOT automatically enrolled. Students who wish to enroll dependents for summer continuation coverage must ALSO submit the Dependent Enrollment Form. Students not enrolled in CampusCare for the spring semester who do not register for the summer semester are not eligible to purchase summer coverage.     ");
