MSW Program Information Sessions
Between October 1 and January 15th all applicants are required to submit the UIC Online Application. After the University has received your application you will receive a link to upload your MSW supplemental application and materials such as transcripts, personal statements and recommendation forms in approximately 2-5 days. To ensure that all supplemental materials are received by the deadline, please submit those documents before 11:59 p.m. CST January 31, 2015.
In order to apply to the MSW program applicants must submit the following items electronically:
- UIC Online Application
- Jane Addams MSW Supplemental Application
- A Resume of your education and employment background
- A personal statement
- Contact information of your references
- ACT Plus Writing, SAT, or Test for Academic Proficiency - for School Concentration Applicants Only
- Concentration Choice - for Advanced Standing and Transfer Students Only

Application Procedures Step-By-Step
Step 1 : Submit the UIC Online application by clicking one of the following links: Domestic applicants | International applicants. When prompted select MSW--Master of Social Work from the menu of degree programs. You must submit this application first to initiate the application process. Once submitted you will be directed to a confirmation page. Approximately 2-5 days after you submit a web application, a Confirmation and Acknowledgment Letter from the UIC Office of Admission and Records will be emailed to inform you that the document upload page is available.
Once you have access to the application document upload page you will proceed to upload the documents described in steps 2 through 7 below. The materials in steps 2 through 7 will be listed under Additional Program Requirements in your checklist. As you upload documents each item can be verified as received by checking the "Received Date" in your application status and the "Status" field which will state "Verified - Correct". A missing received date and any status other than "Verified - Correct" may indicate that you must upload documents again or follow up with the UIC Office of Admissions or Jane Addams College of Social Work. Special Instructions: Due to some technical difficulties the College of Social Work is asking that students discontinue using any Mac/Apple products to submit their application materials. Mac/Apple students have two options to submit their applications.
- They may fill out the forms, print them, scan them and save them to your computer, save them as a Pdf and upload them to their respective sections on the application checklist or
- Students may use a PC to submit their application materials.
Step 3: Submit your resume using this fillable form ONLY. . Complete the form, save it to your computer as a .pdf file, re-open it to verify that the content is saved, and then upload it to your application. Click here for the form. Do not Upload your Personal Resume use the form Provided.
Step 4: Submit your Personal Statement using this fillable form. Complete this form, save it to your computer as a .pdf file, re-open it to verify that the content is saved, and then upload it to your application. Note: You must limit your typed responses to the space allotted in the form. You must upload all the questions at one time as one complete document. Step 5: Submit the contact information for your recommendations/references. Please submit the names and contact information for your references using the reference form on your online application checklist. Once you submit the email addresses for your references the University will automatically send an email to your references requesting that they submit a letter of recommendation electronically.
References should be from people qualified to evaluate your ability to complete the MSW program successfully. Undergraduate college students or recent applicants should seek at least one reference from a college instructor. If you have been employed at least six months, you should seek a reference from a supervisor. Other references may come from volunteer supervisors, college advisors or agency consultants. Do not submit references from friends and family.
Step 6: Submit transcripts Applicants should not send official paper academic credentials by mail. Instead, applicants should request official, sealed academic transcripts from each college or university attended. The credentials must be sent to your home, and must be opened, scanned and saved to your computer or other storage device, and then retrieved later to upload to your application checklist. This step must be followed in order to ensure timely processing of your application and supplemental materials. The credentials once opened become unofficial. Online academic histories will not be accepted.
Once admitted to the College, individuals who accept an offer of admission and confirm plans to attend Jane Addams College of Social Work are required to submit new official transcripts in sealed, unopened envelopes. These new official transcripts are sent directly from your college or university to Jane Addams College of Social Work and the Office of Admission and Records at the University of Illinois at Chicago.
Step 7: ACT Plus Writing, SAT or Test for Academic Proficiency (School social work concentration students only)

Step 8: REQUIRED: Advanced Standing and Transfer Students only. Click here to complete the form to indicate your concentration choices.
