Description
The
Ewing Community Self-care Center is a project of the Chicago Southeast
Diabetes Community Action Coalition, funded by the Centers for Disease
Control and Prevention as part of its CDC-REACH 2010 Initiative.
The Chicago Southeast Diabetes Community Action Coalition is a project
of the Midwest Latino Health Research, Training and Policy Center,
a unit of the Jane Adams College of Social Work at the University
of Illinois at Chicago.
The
Self-care Community Center has been designed as a resource for neighborhood
residents experiencing or at risk of developing diabetes or cardiovascular
disease. Residents will be able to participate in educational programs,
attend support groups, and receive information regarding their health
care needs. The Self-care Community Center will also provide a place
where interested residents, clinicians and activists can come together
and address the community conditions that contribute to the high
incidence of these diseases.
Our
activities focus on delivering disease management and prevention
education, providing health promotion resources and information
that benefit the community, and improving access to health services.
The goal is to help reduce the incidence of diabetes, cardiovascular
disease and other chronic disorders in African American and Latino
communities on the southeast side of the city. and also we hope
that through the centers our information helps to empower the community
to organize and develop networks to focus and work on health issues
they face.

Wellness Programs
Cardiovascular
Project: focus on nutrition and exercise classess

This program provides information and support to participants,
creating an environment in which they can learn the skills that
will enable them to be physically active for the rest of their
lives.

Funding Source: Illinois Department of Public Health (IDPH).
- Program
Content: Consists of a 12 week (one hour per week) or 6 week (two
hours per week) step-by-step program designed to help women make
physical activity a part of daily life.

Textile Workshop
Objectives
Evaluate
the impact of using weaving (a textile art technique) with diabetes
participants as a means to create a social network, positively impact
acceptance of their condition, and increase self-esteem.

Learned
When
designing support groups for persons with chronic diseases, the
intrinsic benefits of artistic expression should not be overlooked.

Offering
free health screenings and consultation for people who can not afford
medical care. Collaboration from Trinity Hospital, and Chicago Family
Health Center (Need to Confirm), Jackson Park Hospital and Centro

Reading Club
Participants
from the community had the opportunity to select a topic, read and
discuss it during 1 hour. We meet once a week at the Center.

Sweet Heart Healthy Dinner
The
purpose of this event is to show the participants (House makers)
the basic ideas to become physically active and start good nutritional
habits. This night event includes presentation of the Sweet Heart
Program, Nutritious dinner and group discussion.
