Introduction

The Community Self-Care Centers have been designed as a resource for neighborhood residents experiencing or at risk of developing diabetes or cardiovascular disease. Residents will be provided an opportunity to participate in educational programs, attend support groups, and receive information regarding their health care needs. The Community Self-Care Centers also provide a place where interested residents, clinicians and community based organizations can come together and address the community conditions that contribute to the high incidence of these diseases.

Major project interests are focused on providing disease management and prevention education, health promotion resources and information that will benefit the community in improving access to health services. The goal is to help reduce the incidence of diabetes, cardiovascular disease and other chronic illnesses in the African American and Latino communities on the southeast side of the city. We hope that our information helps to empower the community to organize and develop networks that will focus on the health issues they face
