SUPPLEMENTARY MATERIALS
Each student must submit a list of activities, a personal essay, and a current unofficial transcript. Depending on the preliminary answers you provide on your application, you may be asked to include other documentation to support your application for particular scholarships.
All supplemental materials must be submitted in pdf format. If you do not have direct access to a scanner, or require assistance in using a scanner, please visit one of the UIC computer labs. Many public libraries also offer access to a scanner. If you require additional assistance, please contact Rachel Leamon (scholarships@las.uic.edu).
FOR ALL APPLICANTS
a. List of Activities
You will need to upload a document containing a list of activities in the following categories, giving each the underlined heading indicated. Please list in chronological order.
- Educational Background: List the high school from which you graduated and any colleges or junior colleges attended and degrees earned. Include dates attended.
- Employment: List employers (if any) since you enrolled at UIC or for the past four years (whichever period is longer). Give the dates of employment and a brief description of duties.
- Honors: List any scholastic honors or other evidence of scholarship received since you enrolled at UIC or during the past four years (whichever period is longer).
- Service or Volunteer Activities: List service activities to UIC or the community since you enrolled at UIC or during the past four years (whichever period is longer).
- Extracurricular Activities: List extracurricular activities, including sports or clubs, since you enrolled at UIC or during the past four years (whichever period is longer).
b. Personal Essay
You will need to upload a double-spaced essay of no more than 500 words describing why you believe you deserve a scholarship. In your essay, specifically include the following:
- Activities, honors, awards, or employment most important or relevant to this application.
- Contributions that you have made, to the UIC community or the community in which you live, while studying at UIC. If you received an award or other recognition for your participation, please also include this information.
- Your personal, professional and academic goals for the coming year and post-graduation.
As you compose your essay, be sure to keep in consideration any specific LAS Scholarship(s) for which you are eligible.
c. Unofficial Transcript
You will need to upload a copy of your current unofficial transcript to your application materials. The report is available through UIC Portal. Login then go to Academics -> Records -> Log Into Student Self-service--Records Menu -> View Academic History -> Submit.
To make a digital copy of your unofficial transcript you have several options:
- If your computer/web browser allows you to print files directly to PDF format, this is the easiest option.
- You can physically print your unofficial transcript and then scan it into a PDF document.
- You can copy and paste the entire contents of your unofficial transcript into a word processing document.

For the Adam Kuhn Scholarship only
List of Medical Schools
If your initial application indicates that you qualify for the Adam Kuhn Scholarship and you would like to be considered for this scholarship, please attach a list of medical schools to which you have applied, or will apply, for admission. Please also indicate your planned specialty, if known.
Additional Personal Essay
If you are eligible for the Adam Kuhn Scholarship, you should include an additional personal essay (no more than 500 words in length) that focuses on your reasons for pursuing a medical education. Please identify your intended field of medical practice and the reasons for choosing that medical specialty.
For the Mario V. & Edith J. Gambardella Student Award only
If you are eligible for the Mario V. and Edith J. Gambardella Award, please include in your personal essay a brief statement about your Italian heritage.
For the PNA-Hugh Hill Scholarship only
Media Internship:
If your initial application indicates that you qualify for the PNA-Hugh Hill Scholarship, please include in your supporting materials:
- A list or description of the coursework you have had in the areas of electronic and/or print media.
- The following information for your internship organization(s):

Graded Writing Sample
If your initial application indicates that you qualify for the Bernard Shaw Prize and you would like to be considered for this scholarship, please submit a graded writing sample from one of your UIC classes. This can be an essay, composition, or paper but must include the instructor’s evaluation.
For the Vietnam Veteran’s Memorial Fund Scholarship only
Proof of Veteran Status
If your initial application indicates that you qualify for the Vietnam Veteran’s Memorial Fund Scholarship and you would like to be considered for this scholarship, please attach to your completed application a copy of the veteran’s DD214 form and a copy of your birth certificate or equivalent proof verifying relation to the veteran.
