Cardew-Mullane Award in Clinical Excellence and Leadership Nomination

The deadline for 2013 nominations is Friday, March 8, 2013. The College of Nursing Alumni Association Awards Committee will select the recipient and the award will be conferred at commencement. If you experience any difficulties with this form, please contact the Alumni Association at 312.996.1736, fax the nomination to 312.996.8066, or send an e-mail to nherman@uic.edu.

Please note! At least two (2) letters of support, one from a clinical instructor, must accompany a nomination. The letters should describe the student's clinical practice and leadership abilities. Please have these letters forwarded to nherman@uic.edu.
