About UsLocationsThere are three centers located in Chicago: Integrated Health Care North, Integrated Health Care South, and Integrated Health Care – Mother’s Project. Integrated Health Care North
Providers Staff Serving the South side, Integrated Health Care South is located in the Thresholds South Building at 749 West 47th Street. Nurse practitioners are available five days a week at both centers for full service primary care including examinations, treatment, counseling, referral and some laboratory testing.
Staff Integrated Health Care – Mother’s Project On the North side is Integrated Health Care located in the Thresholds Mother’s Project. A nurse practitioner is available 1-2 days per week for primary care to young women and their children. Services include examinations, treatment, counseling, referral and some laboratory testing.
