Process overview:
Please note: The Nursing application is a two-step process that involves a university application (referred to as Common Application) and a supplemental application (referred to as the NURSING SUPPLEMENTAL APPLICATION). The university application is required of all non-UIC students, and is managed by the university admissions office. The supplemental application is required of all students, and is managed by the College of Nursing. Both must be correctly completed to be considered for the program.
1. UIC Common Application
The application process varies slightly between students applying from UIC and students applying from other institutions.
Transfer or readmit students must first submit a University Common Application. Current UIC students may directly request the supplemental application materials. Make your selection below:
2. NURSING SUPPLMENTAL APPLICATION
Transfer applicants from community colleges or universities
will receive an email from conapply@nursing.uic.edu with a link to nursing supplemental application within ten days of submitting the UIC transfer application. Do not delete this e-mail. Be sure to check your "spam" and "junk" folders if the e-mail link hasn't arrived in your primary inbox. If you do not receive the nursing supplemental application after ten days please email conapply@uic.edu.
We have found that Yahoo and AOL email accounts aggressively block this email. You may want to create an alternative account with Gmail or another server for your Nursing application.
In preparation for completing the supplemental application, you can update your work and volunteer history and begin working on the following essay questions. Each response should be approximately 200 words:
What experiences have you had with healthcare?
What experiences you have had with being a leader?
Why are you interested in a career in nursing?
Have you participated in any activities to learn more about nursing?
What life experiences have influenced your interest in nursing?
What experiences have prepared you to work with diverse populations (i.e. racial/ethnic, age, gender, socio-economic) seeking health care?
Tell us about some of the challenges or successes in your life that have influenced your academic performance.
Describe a situation in which you overcame a significant obstacle or barrier in achieving your goals (such as academic, economic, language, racial/ethnic, etc.)
2a. Two Electronic Recommendations:
Once you have completed the supplemental application your recommenders will receive an email from conapply@nursing.uic.edu with a link to complete the online recommendation form.
- Hard copies will not be accepted.
Please notify your recommenders that they will be receiving an e-mail from UIC College of Nursing with further instructions.
- Please note: Certain e-mail providers may direct the correspondence to a “junk” or “spam” folder. If the e-mail is never received, it is the applicant’s responsibility to obtain an alternate e-mail address from their recommender and resend the electronic recommendation.
- At least one recommendation should come from someone who can speak to your scholastic ability. The other letter can be from a professional or academic reference. Acceptable references include work and volunteer supervisors. Personal references such as friends, relatives and co-workers will automatically be disregarded. It is the applicant's responsibility to verify that their electronic recommendations have been submitted.
Please click here (.pdf) for a sample of the recommendation form. Please note, recommenders cannot attach a letter of recommendation. For additional information, they should use the "additional comments" field on the form.
Electronic recommendations from previous application cycles are not retained. New electronic recommendation must be submitted for each admissions cycle.
2b. Prerequisite Self Evaluation Form: Word.
Instructions for this form are included in the Nursing Supplemental Application. Use this document as a guide only. Do not mail this document in.
