School Nurse Certificate Program

How to Apply

Students who apply to the School Nurse Certificate are charged e-tuition as it is a fully online program. The current e-tuition rate (2010-2011) is $725 per credit hour.

Application materials must be complete and postmarked by:

November 15 for Spring
April 15 for Summer
July 15 for Fall

Application Materials
A complete application includes all of the following:

1. UIC Graduate Application
Please complete the online University application. There is a $50 non-refundable application fee that can be paid by credit card.

When applying online please make sure to follow these instructions:

First, under APPLICATION TYPE, select Graduate Campus Certificate. Under PLANNED COURSE OF STUDY you should choose: Non-Degree School Nurse (Campus Certificate).

DO NOT apply to the wrong semester or program. Doing so may forfeit your application fee. Please avoid completing this step close to the deadline
so we have time to check your application after it has been submitted for
any errors.

2. Official Transcripts
Official transcripts must be sent from each university and college you have attended. Send all transcripts together in one envelope to the address below. Current and former UIC students do not need to submit UIC transcripts.

3. Nursing Supplemental Application
Within 10 days of submitting your UIC Graduate Application, you will receive an e-mail link to the Nursing Supplemental Application. For this reason, it is imperative that you enter a valid e-mail address when submitting your UIC application.

3a. Two Electronic Letters of Recommendation
Recommendations can be from a supervisor, a certified or master's level nurse who knows the quality of your work, former instructors, etc.

Instructions for submitting the letters are included in the above Nursing Supplemental Application. Hard copies will not be accepted due to the fact that they will be submitted electronically by your references.

3b. Current Resume
You will be able to cut and paste your resume into the Nursing Supplemental Application. Do not mail in your resume.

4. Copy of Current Nursing License
Please mail in with your supporting materials.

5a. ISBE Test of Basic Skills Score Report
Please mail in with your supporting materials. If the ISBE Test of Basic Skills results are not yet available, please include a copy of the registration form identifying the date of the test.

PLEASE NOTE: Documentation of a passing score of the ISBE Test of Basic Skills is required prior to the start of classes.

5b. Grades no lower than "C" or the equivalent
In all professional education and content-area coursework that is needed for certificate.

transcripts will be evaluated for a "C" or better in at least one class that addresses the following four areas:

  • Public Health Nursing
  • Human Growth and Development
  • Social Science
  • Mental Health
