Immunity and Compliance Requirements

Overview

UIC College of Nursing students are required to provide a variety of information in order to promote student and patient safety. Information provided by students may be shared with clinical agencies where students have clinical practica. Depending on your program of study you may be asked to provide immunization/immunity information, background check results, drug screens, HIPAA training and CPR certification. Please see grid below for your program requirements. All information must be submitted before students' registration holds will be lifted.

Students are required to submit compliance records using the online Immunization Tracker as described below.

Deadlines for document submission:  

  • BSN, MS, DNP, PhD and all certificate students: July 1st.
  • GEP students: before orientation in October
UIC On Campus Medical Immunization Requirements

Illinois state law requires proof of certain immunizations for students living on campus. The On Campus Housing Medical Immunization Form, policy and contact information are available at the Office of Medical Immunization Records . Read this information carefully as registration for future terms is not allowed if you are not in compliance. Please print out the form on the link above (Proof of Immunity) and mail or fax it to the address below. Do not mail UIC Medical Immunization Forms to the College of Nursing.

College of Nursing Compliance Requirements

All students in the College of Nursing are required to have:
  • Titers for Measles, Mumps, Rubella, Varicella, and Hepatitis B and necessary vaccination/boosters (Correct blood tests are indicated on the CON Clinical Compliance form)
  • 2-step PPD skin test and current annual PPD skin test or QuantiFERON ®-TB Gold
  • Tetanus/diphtheria vaccination/booster within the past 10 years
  • Annual Flu vaccination

These recommendations are congruent with the guidelines of the Centers for Disease Control and Prevention (CDC) guidelines for health care professionals and according to requirements specified by clinical sites.

College of Nursing Compliance Form must be completed by a student’s health care provider.

All students must meet these requirements prior to their first semester of enrollment.

The McKinley Health Center Immunization Information Form must be completed by all undergraduate and graduate students in Urbana only.

Titers

Positive IgG titers are required for measles, mumps, rubella, and varicella. IgM titers do not fulfill this requirement. A  positive Hepatitis B surface antibody (HepBsAb or antiHepB) is required for Hepatitis B.

Titers are acceptable from any time in the past. The titers must be actual lab reports.

Many health insurance companies do not cover the cost of blood titers. Some students can get them done without charge if they work at a hospital or other healthcare agency. However it is an out-of-pocket expense for most students.
Because of this, we have arranged for special pricing for students to have these tests done:

  • University Health Service, here on campus at UIC. Their contact information and hours are at the top of the titer request form, “Titer Order Form – University Health Service (for students in Chicago)”. Appointments are required.
  • Quest Diagnostics Labs. Students who would like to have titers done through Quest need to DOWNLOAD AND PRINT the form and then submit pre-payment in the form of a money order to the College of Nursing as stated on the form. PERSONAL CHECKS WILL BE RETURNED AND MAY DELAY YOUR CLINICAL PLACEMENT.  Debbie Gabl at buster@uic.edu, phone: 312-996-5741 manages this process.

Negative titers usually require revaccination. Refer to immunity@uic.edu or the FAQ page.

 
CPR Certification

All students must have current CPR certification. Certifications MUST be AHA approved at the level of professional or healthcare worker rescuer, which includes the use of the AED, one and two man rescues for all ages, and the use of external ventilation devices. Classes that meet the CPR requirement include:

American Heart Association(AHA) BLS Healthcare Provider or Advanced Cardiac Life Support or Pediatric Advanced Life Support. American Heart Association instructor status only satisfies the requirement if the instructor status is for BLS or ACLS Healthcare provider. Certification expires two years from the date of issue and must be renewed prior to the expiration date.

Basic CPR classes offered to non-medical individuals are NOT acceptable. Classes that do not meet the requirement include: American Red Cross- all levels and American Heart Association- Heartsaver, Heartsaver CPR, Heartsaver AED, Friends and Family CPR, Heartsaver First Aid.

Below are the preferred CPR trainers for UIC students in Chicago. Certification can be obtained elsewhere as long as the training meets the requirements above. Proof of CPR certification (both sides of the card) should be mailed in or uploaded with your immunization requirements.

On-site Basic Life Support (BLS) for Healthcare Providers will be offered for Chicago BSN and Chicago GEP students by AED & CPR Training and Consulting around the time of orientation (details will be sent directly to you with orientation information). You may register for the class held at UIC or any of the classes listed on the calendar in the link below. The discounted course cost is approximately $35 at the College of Nursing. Alternately, you can attend a Healthcare Provider CPR/BLS class at CPR Associates. Classes are offered Monday through Saturday. The course cost is $30 and textbooks are $15. CPR Associates is located at 2616 W. Peterson Street, Chicago, IL 60659. (773) 973-6933.

FERPA

Please read and sign the Consent to Release Education Records (aka) FERPA form giving the CON permission to share your compliance information with clinical sites or agencies as necessary for the purpose of attesting to your clinical compliance readiness.

HIPAA Training

HIPAA stands for the Health Insurance Portability and Accountability Act of 1996. Students in the BSN and GEP programs will receive HIPAA training at orientation. All other students must provide documentation of HIPAA training. This can be acquired through your place of employment and documented via certificate, employee record or a letter from an employer.

Students can also watch this HIPAA video and sign this form as proof of completion. If necessary, Real Player can be downloaded for free from www.real.com.

RN license

Graduate students must submit a current license.In some cases, a license is also required for the state of the clinical practica if this is different than the student’s license. Students will be notified in this event. The license needs to be uploaded into Immunization Tracker with every renewal. Remember to maintain your CEU’s needed for renewal.

Exemption of Requirement Waiver

Extenuating circumstances, such as an unexpected health or family crisis, may arise and create a situation in which the student needs more time to complete a requirement. The process to request a waiver is explained on the waiver form.  Please give complete information so that a reasonable decision can be made related to the request. Only significant reasons will be considered. The waiver is not to be used in cases of procrastination.

Medical exemptions are to be noted on the College of Nursing Compliance Form and with documentation from the healthcare provider. This should be uploaded onto the corresponding requirement in your Immunization Tracker.

Drug Screen

A 10-panel urine drug screen is required upon admission for students. It may also be required again according to clinical site requirements. Students will be informed of that event as indicated. Students must upload the results(not just the receipt) into their immunization tracker account.

There are two sources for getting the drug screen done. Only these two sources are acceptable. Drug screens from other sources, such as employment are not acceptable. Drug screening costs approximately $45.

Source 1: For students who are close to the UIC Chicago Campus

The urine drug screen may be completed through University Health Service (map) at 835 S. Wolcott, on the UIC Medical Campus. Both the test and the results are obtained from UHS. Results are not mailed out in any form to students. Results are often ready within 72 hours, at which point students will be notified to come in and pick up their results from University Health Service.

Source 2: For students who are far from the UIC Chicago campus

The urine drug screen can be done in your community by ordering the drug screen from Certified Background. Students must use the package code in the Certified Background Package Code table below. Students will be notified by email of the procedure to follow for obtaining a drug screen through Lab Corp or Quest in your community. Students MUST FOLLOW THESE directions carefully.

Background Check

Students are required to have a background check prior to the start of classes (see table above). New international students are an exception. See explanation below.

The College of Nursing has a contract with Certified Background Corporation. The background check must be done through this company and is completed online. Once completed, the background check results are made available to both the student and to the Deans of the College of Nursing. Students should check their report for accuracy. The background check is required once. The cost for the check is approximately $52. Background checks from other agencies will not be accepted.

After ordering your background check, use your Background Check Request Confirmation document to submit to Immunization Tracker. Do not submit your actual background check results. Actual background check results will be rejected on your Immunization Tracker.

International students who are new to US are not required to have a background check for the first year. New international students must submit a signed New International Student Background Check One Year Extension Request Form (see Forms table).

Online Immunization Tracker

The completed College of Nursing Compliance Form and all supporting documents are stored in an online program called Immunization Tracker. It is also managed by Certified Background but created by Magnus Health Portal. Students must purchase and register for their personal Immunization tracker account at www.CertifiedBackground.com. See table below for the correct package code. Note IM in lowercase is “im”. Use uppercase only however when ordering.

Certified Background provides the following services for the College of Nursing: background check, Immunization Tracker documentation management and drug screen testing. The background check and Immunization Tracker are mandatory services for students. The drug screen is  optional only if UHS is used instead for drug screen testing.

Step 2 Attaching Records into your Immunization Tracker account

After setting up your tracker account, the student submits the compliance documents to the requirements in the tracker account. Documents can be uploaded via Self-service uploading method via computer or with assistance from Magnus Health. Both methods require online actions.

A. Self-service uploading method via computer instructions:

  1. Log into your account at https://secure1.magnushealthportal.com
  2. Scan compliance documents into your computer. Save in pdf.
  3. In your Immunization Tracker account, click on “Upload new record”
  4. Browse your files and add it to your tracker account. Repeat til all files are uploaded, named and matched to the requirements.
  5. Save your work.

B. Assisted service uploading method via fax or mail instructions:

  1. Log into your account at https://secure1.magnushealthportal.com
  2. Click on “Fax/Mail new record”.
  3. Download your personalized coversheet.
  4. Fax or mail in compliance documents to addresses below with the coversheet.
  5. Approximately two days later, look for email notification from Magnus Health  that your documents are uploaded.
  6. Log into your account at https://secure1.magnushealthportal.com
  7. Select “Click to complete”.
  8. Proceed with naming and matching your documents to the requirements.
  9. Save your work.

Students must pay a yearly fee for the Student Immunization Tracker service. Students will have the option to renew their account after graduation in order easily accesses their immunization records for potential employers or future educational endeavors. The CON will not be responsible for storing student records. Students also have the option of printing the pdf for their own records.

Finally, students with incomplete documentation will receive weekly e-mails from Magnus Health until they are compliant. If one of your documents expire, please upload the old and new documents so there is a visible history of your compliance throughout the program.
