Clinical Forms
Each clinical site has specific requirements. Please see below to complete your clinical requirement for your clinical site. Please note: If you are at the same site more than once a semester, we still require a set of forms for each clinical rotation.
BSN and GEP students: All clinical forms for you next semester must be submitted to the Phil Zack in the Office of Academic Programs room 524 by the deadline.

MS and DNP students: All clinical forms should be arranged with your faculty advisor. Do not submit forms to the Office of Academic Programs.
Clinical Site Requirements

University of Illinois Medical Center (UIMC)
Please read the "Orientation Packet", sign pages 11,29,31, and 33 and submit them to the Office of Academic Programs (OAP).
You must obtain your UIMC ID Badge on your own prior to beginning your rotation, use the ID Badge Authorization form (which you must get signed by Phil Zack in Room 524) and go to the office listed on the form. Your ID Badge must state your unit and the start and end date of your rotation. At the end of your rotation turn your ID badge into your preceptor or Clinical faculty to destroy. If you have more than one clinical at UIMC, you must obtain a new ID badge for each clinical rotation.
Powerchart training (formerly Gemini) must only be completed once prior to starting a practicum at UIMC. If you have completed Powerchart training in the past, you are exempt from retraining. If you have not had Powerchart training, you must submit a request ASAP. Request Powerchart Training
Advocate Hospitals (includes Christ Hospital, Good Samaritan, Illinois Masonic, and Lutheran General Hospitals)
On the fisrt day of clinical bring the "CareConnection form" to complete it in conjunction with your clinical instructor.
Advocate Illinois Masonic
Inaddition, students going to Illinois Masonic must review the "Student Guidelines", "Education Packet", "Patient Rights Powerpoint", "Patient Safety Powerpoint" and complete the three quizzes. Turn in your completed "Quiz Answer Sheet" to the Office of Academic Programs (OAP) along with the last page of the "Student Guidelines" form. Lastly, please review the "IMMC Handbook".
University of Chicago (U of C)
Read the entire "Orientation Packet", sign the last two pages and submit them to the Office of Academic Programs.
Complete a series of online computer-based training modules and at the end print the certificate and submit it to OAP.
Children’s Memorial Hospital
Complete the “Clinical Skills List”, “Student Competency Test” and the “Certificate of Patient Confidentiality” and submit them to the Office of Academic Programs.
Loyola University
Complete the “Student Nurse Orientation Checklist”, “Confidentiality Agreement”and “Post Test Answer Sheet” (page 81- 84) and submit it to the Office of Academic Programs. Keep the "Evaluation of Student Nurse Clinical Experience" (page 85) until your clinical rotation is complete.
Chicago Department of Public Health (CDPH, TB Control Clinic)
Read the “HIPPA Training Manual” and “What is a public service intern?” . Submit the "HIPAA Compliance" and the “Fingerprint Screening” form to OAP. You will be contacted by OAP to sign up for a fingerprinting date at City Hall one-two months before your rotation begins. When you go to get fingerprinted bring a copy of your driver’s license, “Resume”, “Degree Plan”, “Intern Expectation Form”, “Intern Confidentiality Agreement”, “Resignation Letter”, “Internship Agreement”, and “Hire Certification” forms. Your fingerprint results need to be cleared before you can begin clinical. Result may take 3-6 weeks to process, and will be forwarded to OAP. Fingerprinting, from previous rotations at CDPH or other agencies will not be accepted in lieu of CDPH fingerprinting.

Jesse Brown VA Medical Center
Read "VA Handbook" and sign & submit page 6 to OAP. Complete the “I-9 Employment Eligibility Verification”,"Appointment Affidavitt", "Declaration of Federal Employment", "Application for Health Professions Trainees", "Application Memorandum" and "WOC Letter" and submit it to the Office of Academic Programs. Submit copies of two legal forms of identification: one picture
ID (driver's license or state ID), and either social security card or birth
certificate. Although one of the documents mention notarizing, you are not required to have
any documents notarized. After the submission of you paperwork you will be given further instructions on fingerprinting, and an online training
video.
Northwestern Memorial Hospital
Review the “NMH Safety Self Study” document below. In addition, you are required to do the computer based Powerchart training available on the 3 computers at the first 3 rows of the computer lab in Room B10. Open the “NORTHWESTERN” folder on one of the computers, and click on “PCT_Home.exe” to begin the training. Review the webased powerchart guide book and directions. Login and complete the online modules and quiz at least 2 weeks before your clinical begins.

If students encounter any technical issues during the online quiz, they
can contact Cerner Support at uCernsupport@cerner.com Fill out the “Northwestern Compliance” form when you are done all tasks and turn it in to the Office of Academic Programs along with the "Seasonal Flu Form" and "HR Forms" . Lastly, be prepared to take a quizzes on safety self study during your orientation date at NMH.
Mt. Sinai
Please read the "Clinical Affiliation Handbook" and "Orientation Powerpoint" below. Complete the "Affilliation Agreement" documents and submit to OAP.
NorthShore University HealthSystem
Review "Student Nursing Orientation" powerpoint. Submit "SSN release form" to OAP. You will be contacted before clinicals begin with an EPIC Training ID Number. Once you have your ID number follow the directions under "Epic Training", "Access Instructions", and "Viewing Scores" to complete EPIC training. The training must be completed at least two weeks before the beggining of your clinical.
Glenbrook Hospital
Submit "SSN release form" to OAP. You will be contacted before clinicals begin with an EPIC Training ID Number. Once you have your ID number follow the directions under "Epic Training", "Access Instructions", and "Viewing scores" to complete EPIC training. The training must be completed at least two weeks before the beggining of your clinical.
Shriners Hospital for Children
After your Clinical Instructor has enrolled you in the Shriners database, please follow the instructions on the "HIPAA Medelearn Training Program" information sheet below. Please print out and bring all the course completion certificates with you on your first clinical day. You will then be asked to sign a Workforce agreement.
Northwest Community Hospital
Students should review the "Campus Map", "Welcome Letter" and "Student Guide" below. The "Student Quiz and Signature Sheet" must be completed and turned in to the Office of Academic Programs. In addition, all students should watch the Carelink (NCH's electronic medical record system) tutorial and print the "Quick Guide for Nursing Students" on slide 77 to use as a reference during clinicals.
NCH requires the seasonal flu vaccination. Students at NCH who decide not to receive the vaccine, will be required to wear a surgical mask while in the clinical setting when deemed necessary the hospital epidemiologist.
Macneal
Sign and submit the “ISSP COMPLIANCE AGREEMENT”, "release form", and a copy of your driver’s license or state id to OAP. Macneal also requires all students to have seasonal flu. Make sure that this information is up-to-date in immunization tracker. Also, review the "Orientation Powerpoint" and the "Sharer document".
Cook County (including Stroger Hospital and Fantus Clinic)
Fill out and sign the student sections of the "Certificate of Compliance" and "Computer request form" and submit to OAP. Go to your immunization account and print out a copy of your TB test, Seasonal Flu, and titers (MMR, Varicella, & Hep B titer) and submit to OAP. (If you have every had a positive TB test, Cook County requires a annual chest x-ray. Therefore, you may be required to get a new chest x-ray if your previous one is older than 1 year.)
Weiss Hospital
Sign and submit the “ISSP COMPLIANCE AGREEMENT”, "SSN release form", and a copy of your driver’s license or state id to OAP
Norwegian American Hospital
Sign and submit the “Confidentiality Form ” to OAP
Edwards Hospital
In order to provide Meditech access, Edward Hospital needs specific information listed below. The information will be used to verify user identity by telephone in the event technical support is contacted. The ISS Department requires 10 days to complete the project. Therefore, student should send the following information as early as possible: First name, middle initial and last name, Address, Date of Birth ,expected year of graduation.

Transportation
Clinical sites may be in varied locations and may not always be accessible by public transportation. Arranging transportation to and from clinical sites is the responsibility of the student. Student visiting and parking at clinical locations should be advised to review safety tips.
Immunizations
All clinical sites require current immunizations. If your immunizations are not current you will not be allowed to attend clinical. Please take extra care and make sure that Tuberculosis/PPD (yearly), Tetanus/Diphtheria (every 10 years), valid CPR card (every 2 years) and a negative Drug Screen are not current.
- If your CPR Card is expired or will expire during your clinical you can get re-certified at a discounted rate by signing up for CPR training here. CPR cards expire every two years.
- If your site requires an updated or current Drug Screen you can contact the Office of Health Services to schedule an appointment 312-996-7420.
- If your Tuberculosis/PPD is expired or will expire during your clinical please get a new TB test. TB tests are valid for one year. If you initially recieved a 2 step PPD you can update it yearly with only a 1 step.
All immunization records should be uploaded to your Certified Background/Magnus Health Portal Online Immunization Tracker account. Detailed instructions can be found on the Immunization & Clinical Compliance web page.
