❧The ADVANCE PROGRAM and REGISTRATION MATERIALS provides you with a listing and brief description of all the sessions being offered. The booklet also includes forms for registering for the conference and selecting the sessions you wish to attend.

❧The REGISTRATION FORM is used to register 1-10 delegates, to indicate the special events that they plan to attend and provides the fee structure for the conference. The SESSION SELECTION FORM allows you to indicate your preference for sessions to attend.

❧If you wish to register and choose your sessions on-line, the following electronic form will provide that opportunity. Payment should be made by mail at the time the form is filled out. (As of 2/22/09, this form does not work with INTERNET EXPLORER. Other browsers such as MOZILLA work fine. If you have problems, download the REGISTRATION and SESSION SELECTION FORM above and send it to us via conventional mail.)

❧We have reserved rooms at the Chicago Marriott Downtown at 540 North Michigan Avenue–the heart of Chicago’s Miracle Mile and walking distance to almost everything. Rates are $179 + tax/night for a double or single.

The Marriott is listing less expensive rates on its website than those we have contracted for. There are strict rules about cancellation penalties that you need to be aware of. However, if you are willing to assume that possibility, significant savings are available. Your registration at these lesser rates will still be counted as a part of the number of rooms that we have guaranteed. Check the possibilities. Simply remove the code from the box and indicate the dates for which you would like to register.

❧The hotel is holding rooms at the special rate until April 1, 2009. After that, the reduced rate may not be available.
