Medication Therapy Management
The Multiple Medication Therapy Management Clinic is a pharmacy based clinic to assist patients with multiple chronic medications in the management of their drug therapy. The clinic is for patients on multiple chronic medications, patients who have difficulty managing their medications, patients who have difficulty with adherence, and patients with poor understanding of the medications or chronic disease states. The mission of the clinic is to assist such patients in order to improve or maintain their health and prevent or minimize drug therapy-related problems. Services provided included assisting patients with access and adherence issues, assuring medication continuity of care among all their health care providers and organizations, monitoring for drug therapy response and drug related problems and patient education.
