The Supplemental Application Materials for Fall 2011 Admission - by invitation only
All supplemental application materials must be submitted in one envelope and DELIVERED to the UIC College of Pharmacy Office of Student Affairs. Verify completeness of the supplemental application before submitting. Late or incomplete supplemental applications will not be reviewed regardless of when they are postmarked.

We highly recommend the use of certified mail or delivery confirmation for applicants who want proof of delivery. Supplemental status is updated on PharmCAS as supplemental applications are received and opened. The UIC PharmD Supplemental Application and the UIC PharmD Prerequisite Coursework Form can be saved after they have been completed. Keep a copy for your own records. Only those applying to the PharmD/PhD joint degree program need to complete Step 4.
