Applying to a Degree Program

Application to a degree program at the School of Public Health is through SOPHAS (Schools of Public Health Application Service), a service of the Association of Schools of Public Health, which represents the 40 CEPH accredited schools of public health.  Applicants are advised to carefully read the entire set of instructions for completing the SOPHAS application.  In addition, applicants are advised to print a copy of the following set of UIC SPH application requirements and refer to them as they complete the SOPHAS application.  The following instructions supplement the abbreviated SOPHAS  instructions for 2008-2009  posted at:  http://www.sophas.org/documents/Abbr_instructions.pdf.

UIC School of Public Health supplemental application instructions

UIC requires a non-refundable supplemental application fee ($50 for domestic and $60 for international applicants).  The link below will take you to the payment page which includes further instructions on how and where to submit your payment.   http://www.uic.edu/depts/oar/grad/payment_grad.html  UIC SPH will not review an application until this fee has been paid.

Applicants must submit the supplemental application fee and all required supporting documents no later than the submission deadline published in the designation section of SOPHAS.

Applicants interested in financial assistance must complete a Free Application for Federal Student Aid (FAFSA) which is available online at www.fafsa.ed.gov.

Before you apply

U.S. Transcripts:

Applicants must provide a separate official transcript from every U.S. Institution attended. You must send all official transcripts directly to SOPHAS even if the courses are not required for admission or transferred to another institution. This includes non-degree programs (e.g., summer school) and foreign institutions attended.  

  • Please note:  If course work is in progress, Official transcripts with final grades posted for BS/BA degree will be required upon enrollment.  Student copies of transcripts or copies sent from SOPHAS cannot be accepted. Transfer credit appearing on a transcript cannot be accepted in lieu of an official transcript from the institution where the course work was taken.

Sections (3,4,6,7)  Research and Work Experience, Community and Volunteer Service, Relevant Honors and Awards, Other Relevant Information:
Please answer all questions in the above sections completely.

Section 9   Standardized tests:
MPH degree:  Graduate Record Examination (GRE) scores required for ALL applicants, EXCEPT:

    • MPH applicants with a graduate or professional degree at the doctoral level, e.g., PhD, ScD, MD, DDS, DO, DVM, or JD, from an accredited U.S. or Canadian school;

    • MPH applicants with the above-stated degrees from foreign schools whose applications are sponsored by an accepted referral service, such as AFGRAD or AMIDEAST;

    • Physicians who are licensed to practice in the United States.

    • In exceptional cases, the division director may waive the GRE requirement. If an applicant wishes to be considered for such a waiver, the application must be submitted in sufficient time to take the GRE and have the scores sent to the School of Public Health by the application deadline, in the event that the request for waiver is denied. A cover letter must state the basis for the request for a waiver.

    • NOTE: The USMLE and ECFMG will not be considered in lieu of the GRE.

DrPH degree: Graduate Record Examination (GRE ) Scores not required.


MS/PhD degrees:  Graduate Record Examination (GRE) scores required for all applicants. 


MHA degree: Graduate Record Examination (GRE ) Scores, or equivalent required for all applicants.


MS in Clinical and Translational Science degree:  MS applicants with a graduate or professional degree at the doctoral level (e.g., PhD, ScD, MD, DDS, DNP, DO, DPT, DVM, PharmD) from an accredited U.S. or Canadian school or who are licensed to practice in the United States are exempt from the GRE requirement, but must instead submit scores for the appropriate exams in their profession such as the USMLE for physicians, National Board Dental Examinations Part I for dentists, and NPTE for physical therapists. (For example, current medical residents would submit scores for Part I and Part II of the USMLE. Medical fellows must submit scores for Part I, Part II, and Part III of the USMLE.)


Test Score Codes:

Official, certified or attested copies of the following test scores must be sent directly to the University of Illinois, School of Public Health:

    • GRE – Institution Code: 1851-0616

    • MCAT – (UIC SPH will accept an unofficial copy of the MCAT)

    • TOEFL – Institution Code: 1851

    • GMAT- Institution code: H00-Z4-88

    • LSAT – provide official or certified copy from the testing service

    • DAT – provide official or certified copy from the testing service

The following official test scores may also be sent to SOPHAS.  For some tests, you will be able to use SOPHAS as a designation when you take a standardized test.  Below is a current list of tests that will send scores directly to SOPHAS; please check back periodically for updates.

Section 12  Curriculum Vitae:
Curriculum Vitae/Resumes are required by UIC.  Please upload in the CV/resume section of the SOPHAS application.

Section 13   Statement of Purpose and Objectives:
Provide a personal essay in SOPHAS describing your past education, experience, and current professional career objectives. You are encouraged to comment on any or all of the following: plans you have to use your education and training; the needs and/or challenges you perceive as important in your field of study; and any personal qualities, characteristics, and skills you believe will enable you to be successful in your chosen field of study.

Applicants to the MPH program who are interested in applying to the Peace Corps Master’s International Program should indicate their interest in this section of the SOPHAS application.

Section 14   References:
Provide 3 letters of recommendation from persons qualified to assess your academic work; clinical, public health, or professional experiences; or leadership potential in public health. Letters of recommendation should be provided directly to SOPHAS in on –line or in paper format.  Please note:  Electronic Submission is preferred. 

Section 15  Designations:
UIC SPH permits up to two (2) designations per applicant.    You should identify the ranking order of your designations.   UIC requires applicants to complete a few additional questions.  After the designations are applied, applicants must follow the link located above application type in order to complete the additional questions required by UIC.

 

Applying for Credit Nondegree Status

Application for credit nondegree status at the School of Public Health is through the University of Illinois at Chicago's online application system. Applicants are advised to carefully follow each step in the application process as mistakes can delay the processing of your application. Students are encouraged to print out the following supplemental information before proceeding to the online application at:  http://www.uic.edu/depts/oar/grad/apply_grad.html.

How to Apply: Online Application

  1. Visit: www.uic.edu/depts/oar/grad/apply_grad.html
  2. Scroll down to step one under the “Application Steps and Tips” heading.
  3. Click on “Apply Online – Graduate Domestic” or “Apply Online – Graduate International” based on your student status. The admissions login screen will appear.
  4. Click on the "First time user account creation" link below the "login" button.
  5. Create a Login ID and a six-digit password for your application account and click on "login".
  6. Select the application type "Graduate Non-Degree" and click on "continue".
  7. Select the appropriate admission term and type your first, middle and last name and click on "Fill out Application".
  8. The "Application Checklist" screen will appear with a number of sections to be completed. You will need to visit each of these sections one by one and complete all necessary fields.
  9. On the "Planned Course of Study" screen, you will see “Non-Degree Unassigned Graduate” listed as your major. Click "Change this Program".

  10. The "Select your major" drop down menu will appear. Select one of the following public Health Non degree options:
    1. Non-Degree Public Health MPH –UIC
    2. Non Degree Public Health –Public Health Informatics (UIC)
    3. Non Degree Healthcare Administration or
    4. Non Degree Public Health Community Health - Peoria
    5. Non-Degree MS in Clinical and Translational Science
  11. Once you have filled out all the sections on the Application Checklist, click on "Application is Complete" to submit your application.

    1. If you wish to save your application and complete it at another time, click on "Finish Later" from the Application Checklist screen.
    2. To return to your saved application, follow steps 1and 2 from above and sign in to the Admissions Login screen using the Login ID and password that you created previously.
  12. Follow the on-screen instructions and submit your non-refundable application fee ($50 for both domestic and international applicants).
  13. Once you have clicked on the "Application is Complete" button and your application and application fee have been submitted, the following programs request that you send an email notice to the School of Public Health stating that your application has been submitted online:
    1. Non Degree –Public Health Informatics (UIC) email:  PHI@uic.edu

  14. Make sure to turn in all required supplemental documents by the application deadline.

All School of Public Health Credit Nondegree options require the applicant to submit: 

Prior Degrees: A baccalaureate or its equivalent from an accredited college or university. Proof of the degree must be submitted with the application.

Tests Required: Applicants whose native language is not English must take the Test of English as a Foreign Language (TOEFL). The test score cannot be more than two years old. A minimum score of 550 (paper-based test) or 213 (computer-based test) is required by the Graduate College; many departments have a higher minimum. UIC's Institutional Code is 1851. No other tests are required by the School. 

Domestic Applicants: A $50.00 check or money order made payable to the University of Illinois for the nonrefundable application fee.

International Applicants: A $50.00 check or money order made payable to the University of Illinois for the nonrefundable application fee.

Other Requirements: International students who require certification of admission (I-20 or IAP-66) sent to the Immigration and Naturalization Service will not be admitted as nondegree students.

Application to a Certificate Program of the School of Public Health  is through the University of Illinois at Chicago’s online application system.
