  This session provides participants with skills and knowledge for developing and maintaining partnerships among families, youth, and providers to assist youth involved in transition. Presenters highlight the need for youth and young adults to be nurtured, supported, and empowered to make life decisions for all aspects of adult living based upon what they want.
