Parent answers the individual survey items in quality measure 2 in the following way: 11a- Yes, 11b- No, 11c- Yes, 11d- No
1) Recode: Recode each response to a 0-100 values:

2) Score Quality Measure: Average Proportion of “Yes” Responses
Calculate the average proportion answering “yes” across the 4 questions.
