Faye Eldar, M.Ed., Family Liaison, Quality Community Managed Care Project: Phase II, located at the Center for the Advancement of Distance Education, School of Public Health, University of Illinois at Chicago.

Louis Terravova, M.S. is the Program Services Manager of the Division of Specialized Care for Children, the Title V program in Illinois. He has been actively involved in a Transition project during the past year.

Jane Gaulke is the mother of a 21 year old son, Tim, who has a disability and is transitioning to adult services.
