the AAP Committee on Children with Disabilties:"The
Role of the pediatrician in Transitioning Children and

in the transition process, emphasizing the need to provide
anticipatory guidance, foster self-advocacy and become
knowledgeable regarding federal legislation relevant to
transition and vocational services.

Includes a Healthy Ready to Work project. Website has
many helpful documents to download- including Life Maps
for all age groups, Teaching Sheets on healthcare transitions,

website features a downloadable brochure for families
of CSHCN: "Attention- Families of Teenagers with
Special Needs", a down-to-earth guide to access to
health care and government programs related to transition.
Written by the Family Coordinator of the QCMC2 Project,

Illinois) but is designed to be used as a template for
other states to follow.
