This is the official website of the Health and Ready to Work initiative, part of the HRSA Maternal and Child Health Bureau, Division of Services for Children and Youth with Special Health Care Needs.

The site includes extensive publications and handouts on all transition topics related to YSCHN, including health care transition and health insurance issues. The site provides links to all HTRW projects nationwide, along with other transition projects and resources. Include a video clip on HRTW, listing of HRTW Network members, upcoming meetings and much more. There are also links to sign up for two national transition listservs, the MCHB HTRW listserv and the Health Care and Transition for Youth mailing list.

The website focuses on transition as a developmental process, with guidelines for "transition training" for children and youth with special health care needs of all ages. Includes specific information for health care providers, educators, families, and teens and young adults. Offers free "Transition Timelines" chart and many other helpful materials.

Kids as Self Advocates

The website of the consumer organization for YSCHN affiliated with Family Voices, a national grassroots organizations of families of CSHCN. Offers support and peer contacts, with a self-advocacy focus.

American Academy of Pediatrics

The AAP website provides a link to the document produced by the AAP Committee on Children with Disabilties:"The Role of the pediatrician in Transitioning Children and Adolescents with Developmental Disabilities from School to Work or College". Promotes the role of the pediatrician in the transition process, emphasizing the need to provide anticipatory guidance, foster self-advocacy and become knowledgeable regarding federal legislation relevant to transition and vocational services.

Family Village is the premier site for comprehensive, on-line resources for all topics related to children and youth with special health care needs. The transition section is another example of the excellence and ease of use of this site. Extensive listings of articles, mailing lists and websites covering all aspects of transition.

Kentucky Commission for CSHCN

Site of the Kentucky Commission for CSHCN, the Title V agency. Includes a Healthy Ready to Work project. Website has many helpful documents to download- including Life Maps for all age groups, Teaching Sheets on healthcare transitions, materials specifically for young adults.

Illinois Chapter, American Academy of Pediatrics

This website features a downloadable brochure for families of CSHCN: "Attention- Families of Teenagers with Special Needs", a down-to-earth guide to access to health care and government programs related to transition. Written by the Family Coordinator of the QCMC2 Project, it is contains state-specific contact information (for Illinois) but is designed to be used as a template for other states to follow.
