Admission Requirements

1. TRANSCRIPTS - An official copy of the baccalaureate transcript indicating the earned degree(s). International academic documents that are evaluated by a professional credential evaluation service that holds membership in the National Association of Credential Evaluation Services (NACES) are also acceptable. While a grade point average (GPA) of 2.75 is required, a GPA close to 3.0 (on a 4.0 scale) is recommended.

2. TOEFL SCORES - A copy of TOEFL scores taken within the last 2 years (applies to international students only who have not attended a US institution of higher learning for at least 2 years). Minimum scores required: paper-based TOEFL = 550, computer-based TOEFL = 213 [or new TOEFL minimum total score of 80, with subscores of 21 (Writing), 20 (Speaking), 17 (Listening), 19 (Reading)].

3. PERSONAL STATEMENT AND RESUME/CV – Either can be submitted electronically to dbrand@uic.edu or sent via postal mail to the address listed below.

How to Apply: Online Application

1. Visit: www.uic.edu/depts/oar/grad/apply_grad.html

2. Scroll down to step one under the “Application Steps and Tips” heading.

3. Click on “Apply Online – Graduate Domestic” or “Apply Online – Graduate International” based on your student status. The admissions login screen will appear.

4. Click on the "First time user account creation" link below the "login" button.

5. Create a Login ID and a six-digit password for your application account and click on "login".

6. Select the application type "Graduate Non-Degree" and click on "continue".

7. Select the appropriate admission term and type your first, middle and last name and click on "Fill out Application".

8. The "Application Checklist" screen will appear with a number of sections to be completed. You will need to visit each of these sections one by one and complete all necessary fields.

9. On the "Planned Course of Study" screen, you will see “Non-Degree Unassigned Graduate” listed as your major. Click "Change this Program".

10. The "Select your major" drop down menu will appear. Please select “Non-Degree Pub Hlth Rckf(Cert) -UIC

11. Once you have filled out all the sections on the Application Checklist, click on "Application is Complete" to submit your application.

1. If you wish to save your application and complete it at another time, click on "Finish Later" from the Application Checklist screen.

2. To return to your saved application, follow steps 1 and 2 from above and sign in to the Admissions Login screen using the Login ID and password that you created previously.

12. Follow the on-screen instructions and submit your non-refundable application fee ($50 for domestic applicants, $60 for international applicants).

13. Once you have clicked on the "Application is Complete" button and your application and application fee have been submitted, please send an e-mail to dbrand@uic.edu stating that your application has been submitted online.

14. Make sure to turn in all of your supplemental documents (listed above).

1. Transcripts and TOEFL scores should be submitted to UIC's School of Public Health, Division of Community Health Sciences.

2. Personal statement and resume/CV may be submitted electronically by sending an e-mail to dbrand@uic.edu or via postal mail to UIC's School of Public Health, Division of Community Health Sciences.

Tuition & Fees
Tuition and fees for the Rockford certificate program is assessed at the graduate level tuition rate set by the University Of Illinois Board Of Trustees annually.  Assessment is based upon student level, residency status, curriculum/college and total number of hours enrolled for the term.  Tuition and fees are subject to change without notice any time prior to the first day of instruction.  For more information go to   http://www.uic.edu/depts/oar/grad/tuition_grad.html

Changing from Non-degree to Degree Program
Admission as a certificate student does not guarantee admission to any of the School of Public Health programs. Students may transfer all credits earned in the certificate program towards the fulfillment of the MPH (Master of Public Health) or the MS (Master of Sciences in Public Health) degree requirements in the event they apply and are admitted as degree candidates (see SPH admission requirements at: www.uic.edu/sph/eohs_admissions.htm. To transfer credits, the student must earn a minimum letter grade of “B” or above in all certificate courses. Students must attain a 3.00 grade point average (on a 4.00 point scale) to receive Public Health Practice Campus Certificate at Rockford.

Transfer Credit
Students cannot transfer any coursework from another institution into the certificate.
