Admission Requirements
A baccalaureate degree from an accredited university is the minimum requirement for admission to the EHI certificate program. Application to the University of Illinois at Chicago (UIC) is required for admission to the EHI certificate program. Students must apply by completing the Graduate Certificate Program online application. Besides submitting a completed application form and non-refundable application fee, applicants must submit the following information and documentation to UIC's School of Public Health, Division of Environmental and Occupational Health Sciences (address below) in order to be considered for admission to the Environmental Health Informatics certificate program.

1. TRANSCRIPTS - An official copy of the baccalaureate transcript indicating the earned degree(s). While a grade point average (GPA) of 2.75 is required, a GPA close to 3.0 (on a 4.0 scale) is recommended.

2. PERSONAL STATEMENT - A concise typed statement of the student's reason for seeking education in environmental health informatics. Please be sure to include your full name at the top of your personal statement.

3. CURRENT RESUME OR CURRICULUM VITAE

Additional Information for International Applicants

1. International academic documents that are evaluated by a professional credential evaluation service that holds membership in the National Association of Credential Evaluation Services (NACES), in lieu of transcripts, are also acceptable.

2. TOEFL SCORES - A copy of TOEFL scores taken within the last 2 years (applies to international students only who have not attended a US institution of higher learning for at least 2 years). Minimum scores required: paper-based TOEFL = 550, computer-based TOEFL = 213 [or new TOEFL minimum total score of 80, with subscores of 21 (Writing), 20 (Speaking), 17 (Listening), 19 (Reading)]. TOEFL scores can be sent directly to UIC from the testing service. UIC's institution code for the TOEFL is 1851.

Application Instructions

1. Visit: http://www.uic.edu/depts/oar/grad/apply_grad_certificate.html


2. Scroll down to step 3b under the "Application Steps and Tips" heading.


3. Click on "Apply Online to Certificate Programs.” The admissions login screen will appear.


4. Click on the "First time user account creation" link below the "login" button.


5. Create a Login ID and a six-digit password for your application account and click on "login".


6. Select the application type "Graduate Campus Certificate" and click on "continue."


7. Select the appropriate admission term and type your first, middle and last name and click on "Fill out Application".


8. The "Application Checklist" screen will appear with a number of sections to be completed. You will need to visit each of these sections one by one and complete all necessary fields.


9. On the "Planned Course of Study" screen, the "Select your Program" drop down menu will appear. Choose "Non-Degree Environ Hlth Informatics (Campus Cert)" and click "Continue."


10. Click the “Enter Other Majors, Minors and Concentrations” button to indicate your specific program of study.


11. The “Concentration” drop down menu will appear. Choose “Environmental Health Informatics” and click “Continue.”


12. Once you have filled out all the sections on the Application Checklist, click on "Application is Complete" to submit your application.
    
    
    1. If you wish to save your application and complete it at another time, click on "Finish Later" from the Application Checklist screen.
    
    
    2. To return to your saved application, follow steps 1 and 2 from above and sign in to the Admissions Login screen using the Login ID and password that you created previously.
    
    
    3. Click on the appropriate admission term for the “Graduate Campus Certificate” application under the "Applications in Progress" heading.
    
    
13. Follow the on-screen instructions and submit your $50 non-refundable application fee.
    


14. Once you have clicked on the "Application is Complete" button and your application and application fee have been submitted, please send an e-mail to ehi@uic.edu stating that your application has been submitted online.


15. Make sure to turn in all of your supplemental documents (listed above).
    
    
    1. Transcripts and TOEFL scores should be submitted to UIC's School of Public Health, Division of Environmental and Occupational Health Sciences.
    
    
    2. Your personal statement as well as your resume/curriculum vitae may be submitted electronically by sending an e-mail to ehi@uic.edu. These documents may also be submitted via postal mail to UIC's School of Public Health, Division of Environmental and Occupational Health Sciences.

Tuition and Fees
Tuition and fees for the EHI certificate program are currently $695 per credit hour.
Note: Tuition and fees are subject to change without notice any time prior to the first day of instruction.

Additional Information

Changing from Certificate to Degree Program
Admission as a certificate student does not guarantee admission to any of the School of Public Health degree seeking programs. Students may transfer credits earned in the certificate program towards the fulfillment of the UIC MPH (Master of Public Health) degree requirements in the event they apply and are admitted as degree candidates (see SPH admission requirements). To transfer credits, the student must earn a minimum letter grade of "B" or above in all the Environmental Health Informatics certificate courses. Students must attain a 3.00 grade point average (on a 4.00 point scale) to receive the EHI certificate.

Transfer Credit
Students cannot transfer any coursework from another institution into the certificate program.
