FULL DISCLOSURE OF FACULTY FINANCIAL INTERESTS OR RELATIONSHIPS
It is the policy of UIC that any faculty (speaker) who makes a presentation at a program designated for AMA Physician's Recognition Award (PRA) Category 1 or 2 credit, must disclose any financial interest or other relationship (i.e., grants, research support, consultant, honoraia) that faculty member has with the manufacturer(s) of any commerical product(s) that may be discussed in the educational presentation.
UIC does not imply that such financial interests or relationships would prevent the faculty (speaker) from making a presentation. However, it is imperative that such financial interests or relationships be identified by the faculty so that participants at the CME activity may have these facts fully disclosed prior to the presentation, and may form their own judgments about the presentation. Activity Directors are required to document how disclosure was made. Participants will be asked to indicate on their evaluations whether disclosure was made.
In keeping with this policy, the faculty/presenter is required to sign the following disclosure statement. [Faculty/presenter should select link to open the disclosure statement and print a copy to complete, sign, and return to the address indicated on the bottom of the statement.]
