Online applications are now being accepted for the Summer 2008 Semester.
There is both a SOPHAS Form and a UIC SPH Supplemental Form. These forms, together with the necessary instructions, can be found using the link below. Please note that there is a fee associated with each form.
On the SOPHAS Form, you will eventually see a School Designations panel. When you click the "+" button on this screen, you will then be asked to select a school. Once you choose University of Illinois at Chicago School of Public Health, you will be requested to indicate a Degree - Program / Department. For this option, please select MPH - Health Policy and Administration-20FY4021MPHU.
Please write MPH under Degree and HPA/Public Health Informatics under Division/Program in the table at the top of the UIC SPH Supplemental Application Form.

In order to receive full consideration, all forms and associated documents must be received by March 15, 2008 for Summer 2008 Semester and May 15, 2008 for Fall 2008 Semester.
