In order to make the transition as
easy as possible, we have compiled some information
for our students who have recently come from Louisiana
and other areas affected by Hurricane Katrina.

This form
(click above to download) will allow us to understand
what you need. When you have completed it, please return
it to the Office of Student Affairs in the SPH-PI building,
1603 W. Taylor, room 179, attention: Toni Flynn.

Photo
ID and U-Pass
Photo IDs:
You may get
your photo ID at the Student Services Building (SSB),
1200 W. Harrison. If your registration has not been
processed yet, please contact Deborah
Meiselman (312-996-4731) and she will give you a
letter requesting that you be given one.
U-Pass:
After you
get your Photo ID, go to the U-Pass office, which is
within the Photo ID Office. It is in the Campus Auxiliary
Office Services Administration Office in the SSB, room
2560. You must have your photo ID before getting the
U-Pass.
What is the
U-Pass?
Check out
the website: http://www.vcsa.uic.edu/MainSite/departments/Upass/home/.
It is a pass to use on all CTA buses, trains and PACE
busses. This is the major public transportation in Chicago
and surrounding areas. It is not valid on the Metra.
Also, the tuition and fee waiver will include the U-Pass
fee. The U-Pass is typically for students registered
for 12 credit hours or more. The University has spoken
with the CTA to waive this requirement for Katrina-affected
students. If you ask someone and they do not know what

Computing
and Registration
You have
to take the following steps below to get set up at UIC.
- Establishing your Net ID:
The NetID is REQUIRED! It allows you to register and
provides you access to campus-specific sites and services,
such as email, use of computer lab equipment, etc.
You will need to establish your NetID to establish
an Enterprise ID, so it is important to set up your
NetID first.
- Go to http://activatenetid.accc.uic.edu
- Follow the instructions listed for establishing
a NetID and password.
- Be sure to remember your password. If you have
any problems establishing your NetID and password,

- Activate your UIC email account – it’s
mandatory

Why you need to activate your UIC email:
- Access to SPH listservs
- Access to any UIC public lab on campus
- Network Services Kit (NSKIT) usage
- You will be able to access the university system
and the Internet from an off-campus site through
dial-up telephone lines; no additional Internet
provider is necessary.
- Online course access
- All MPH students will take at least one online
course during their academic career at SPH. In
order to access the course, one must establish
a UIC account.
- Access to thousands of online journals through
the e-library.
- To keep updated on university happenings.
- YOU CANNOT REGISTER WITHOUT IT! If you'd prefer
to use an alternative email account (e.g., a hotmail
or yahoo address, rather than the UIC account)
you should set up the mail forwarding mechanism
for your UIC email address. To do this, visit
emails tools: http://www.uic.edu/depts/accc/home/EMAIL.html.
If you are an international student and do not
have a Social Security number yet, you may still
get your netid. Go to the computer lab in person
and talk to the staff there. Go to the Science
and Engineering Laboratory (SEL) at 950 S. Halsted,
on Halsted Street between Taylor and Polk, Room
2267. The Campus Shuttle should take you there.

What is the
CSO?
The Client
Services Office (CSO), part of UIC's Academic
Computing and Communications Center, addresses the
computer support needs of the UIC community. Its aim
is to provide a central location for software support
and sales as well as general consulting. It does not
offer help with class assignments. It tries to help
you resolve problems with software that the office sells
or computer equipment that is located in any ACCC computer
lab on campus.
The main
office is located in room 2267 SEL
(Science and Engineering Laboratories). Office hours
are 9-5 Monday through Friday, with
the exception of Wednesday when the office closes
at 4.
Establish
your Enterprise ID and Password
The Enterprise
ID allows you access to UI-Integrate Self-Service -
the University-wide administrative system for official
records, e.g., Student Services, Financial Aid, Billing,
and other services.
- Go to: https://apps.uillinois.edu
and select the Student & Faculty Self-Service
link. Next, select University of Illinois at Chicago
(UIC) link, scroll down, and follow instructions for
first-time users. You will then be prompted for your
NetID. When you type in your NetID, add "@uic.edu"
at the end. For example, if your UIC NetID is joan,
enter joan@uic.edu.
- Click Enter. You will be prompted for your NetID
password (the one you established in the last step).
- You will be provided your Enterprise ID.
- NOTE: In many cases your Enterprise ID will be the
same as your NetID. If your NetID is different from
your Enterprise ID, you will need to know both depending
on which system you wish to access.
- Once your Enterprise ID is displayed, you will need
to establish an Enterprise ID password. There are
specific guidelines for setting your password. Please
follow the guidelines carefully.
- Please remember your Enterprise ID and password.
You are now ready to register! If you have any problems
establishing your Enterprise ID and password and/or
registering for classes, contact AITS Help Desk at

Registration
Since you
are all here after the regular registration deadlines
you should register through your academic coordinator
(see below). If you have not already
done so, please see your academic coordinator to register
for classes. You may view the School of Public Health
classes being offered this semester at http://apr.sph.uic.edu/str/courseschedule/course_fall.asp.

them available, but are unsure what to take, please

To see if
your registration has been processed:
- Using a web browser, connect to https://apps.uillinois.edu.
- Choose the Web Self-Service option.
- Select your campus (UIC).
- Log in using the Enterprise ID and password established
previously.
- To register: click "Registration & Records,"
then click "Registration," then click on
"Look up or Select Classes," then find the
class you wish to register for by subject, course
number, etc.
- To view registration: click "Registration &
Records," then click either "student schedule
detail" or "student schedule week at a glance."

We know this
is a lot of information, but please review the SPH Student
Handbook available online at http://www.uic.edu/sph/shandbook.htm.
This document will answer a lot of your questions.
Register
for Student Listservs
- There are 2 SPH student listservs, SPHNEWS and
SPHVIEWS. SPHNEWS is MANDATORY! This is where important
notices and deadlines will be sent from faculty and
staff at the school. You will be held responsible
for all information on the listserv.
- SPHVIEWS is an optional listserv which we highly
recommend joining. This is where students can post
to each other. Job opportunities are posted here,
as well as events and other information. Once you
establish your UIC email, using this email send a
message to listserv@listserv.uic.edu
and in the body of the message type "subscribe
sphnews" without using the quotation marks. Please
be sure to type it in the body of the email and not
the subject. To subscribe to SPHVIEWS, send a separate
email to listserv@listserv.uic.edu
and type in the body of the email "subscribe
sphviews" without using the quotation marks.
You will be asked to confirm this request by replying
to the verification email that will be sent to you
and typing "ok" without the quotation marks
in the body of the email. If you have any difficulty,
please contact Gwen Slaughter at gwens@uic.edu.
To get your
book vouchers from the African-American
Academic Network, you must be registered in the
Banner system. You also must have your UIC ID. So please
follow the above steps as soon as possible.
Official
SPH orientation packets are available in Toni Flynn's
office (Room 179). They contain important information
which may be helpful to you.

Tuition
and Fees
The University
has agreed to waive all tuition and fees. This waiver
will apply for the Fall 2005 semester only. It will
include all fees, even the U-Pass. It is not yet clear
if the waiver covers health insurance or not. You will
all be automatically covered by UIC health insurance.

Money
If you need
extra money:
Contact the
Office of Student Financial Aid at 312-996-5563 or email
money@uic.edu. (It
is not clear exactly what they have available, but they
encouraged Katrina-affected students to call them if
they needed extra funds.)
Book
Vouchers
Get vouchers
for your textbooks from the African-American Academic
Network (AAAN) at the Student Services Building (SSB),
1200 W. Harrison. While you are at the SSB, get your
photo ID, apply for your U-Pass and see the Office of
International Services (OIS) if you are an international
student.

Healthcare
Health
Insurance: CampusCare
Check out
the website at http://www.uic.edu/hsc/campuscare/chicago/.
You will be automatically enrolled unless you have other
insurance, and then you can waive it. The waiver form
is available on the website. The deadline to waive health
insurance is September 18, 2005. All you need to do
to waive the insurance is show proof of other insurance
and fill out their form.

International
Students
If you have
not already done so, please stop by the Office of International
Services (OIS) at the Student Services Builidng (SSB),
1200 W. Harrison. This is necessary to make sure your
status is transferred from Tulane to UIC and that you
will maintain legal status.
Also, all
international students must be registered for 12 credit
hours. If you feel you cannot take on that many courses,
talk to your professors about possibly taking an incomplete
so that you can maintain legal status. We are working
with OIS, who is working with the Federal government,
to see if we can make accommodations for the hurricane-affected
students.

Buddy
System
We are setting
up a buddy system. We would like to provide each Tulane
student with three names (one faculty, one staff, and
one student), phone numbers, and email addresses. The
purpose of the buddy system is to have three people
the student can call on day or night, weekend or weekday,
for any reason. For more information, or to let us know

The central American Red Cross office provides information
on how to receive aid in different parts of the city.

This service center is open 24 hours; you can receive
information from 7 am to 11 pm. The center can give
you information on clothing, shelter, and American Red
Cross debit cards. Representatives from agencies like

The counseling center on campus is open and accepting
appointments from transferring students as long as they
are registered. Students can call the center to set

Tulane
University
The Tulane website is active and posts daily. They are
urging their students to contact them via email if they
are having financial aid issues.
Free
Application for Federal Student Aid (FAFSA)
Host schools will process students' financial aid information
as they would for a regular student. Financial aid will
be based on the student's FAFSA. However, if the FAFSA
does not have the new school on it, the new school should
be added.
