Checklist

Everything you should have done before classes begin:

Enrollment Confirmation & Tuition Deposit

In order to secure your admission for the semester, a non-refundable tuition deposit of $150 is required. Checks should be made out to the University of Illinois and mailed to Ms. Antonia Flynn, Coordinator of Admissions, UIC School of Public Health (MC 923), 1603 W. Taylor Street, Room 179, Chicago, IL 60612. The deposit is due within two weeks of your notice of acceptance from the School of Public Health. You must also email your acceptance of admission to the academic coordinator for your division. Upon admission the deposit will be credited to your first tuition bill.



Registration

You will not be able to register until you receive your official acceptance letter from the University of Illinois at Chicago’s Office of Admissions. The acceptance letter includes the official University Identification Number (UIN) assigned to you. This letter is usually sent 2-3 weeks after you receive your School of Public Health acceptance.

  • Before registering you must meet with your advisor to develop a program plan. After meeting with your advisor he or she will remove an “advising hold” that has been placed on your account. (It takes 24 hours to remove the hold). You will be notified who your advisor is within a month (at a minimum) before classes begin.

  • International Students: Certain categories of international students may have a registration hold (also called a SEVIS hold) placed on their record. If you fall within a category for which a SEVIS registration hold has been placed, you must report to the Office of International Services (2160 SSB) to clear the hold. You can check the UIC Office of International Services website and the U.S. Immigration and Customs Information webpage for more information about this requirement.

  • You can register for classes once you have established your NetID and Enterprise ID. You will need a Net ID to access campus-specific sites and services such as your UIC e-mail account and Blackboard. You will need an EnterpriseID to access the university-wide administrative system. For example, you will use your Enterprise ID to register for classes, make address changes, access your student account, review grades, and request transcripts. Instructions for establishing these IDs can be found on the UIC Web for Student site: http://osssorawebprod2.admin.uillinois.edu/webforstudent/wfs3.as.

  • Students may register for classes using the UIC Web for Student system (accessible through Quick Links on the UIC home page at www.uic.edu. An SPH course schedule may be found at http://www.uic.edu/sph/course_schedules.htm.

Get Your Photo ID

By the first week of class you will need to get a photo ID (I-card). New students are able to go to the Student Center West located at 828 South Wolcott, Suite 241 to get your I-Card. You must show a photo ID and evidence of registration for classes, which can be printed from the Student Access System. The I-Card office hours are Monday-Friday from 9 am to 1:00pm (closed for lunch from 1-2) and from 2:00 pm- 5:00 pm.



University Insurance

All registered students are automatically enrolled in Campus Care Health Insurance. There is no enrollment form to complete. Students with equivalent health insurance coverage can opt out of the Campus Care Program by submitting the online Waiver Form within designated time frames. The form only needs to be submitted once while you attend UIC. The waiver form and more information can be found at: http://www.uic.edu/hsc/campuscare/chicago/.



Register for the SPH listservs

There are 2 student listservs SPHNEWS and SPHVIEWS.

  • SPHNEWS is MANDATORY! This is where important notices and deadlines will be sent from faculty and staff at the school. You will be held responsible for all information on the listserv. To subscribe to SPHNEWS send an email to listserv@listserv.uic.edu and type in the body of the email “subscribe sphnews” without using the quotation marks. You will be asked to confirm this request by replying to the verification email that will be sent to you and typing “ok” without the quotation marks in the body of the email. If you have any difficulty, please contact Gwen Slaughter at gwens@uic.edu.

  • SPHVIEWS is an optional listserv which we highly recommend being a part of. This is where students can post to each other. Job opportunities are frequently posted here as well as events and other information. Once you establish your UIC email, using this email send a separate message to listserv@listserv.uic.edu and in the body of the message type “subscribe sphviews” without using the quotation marks. Please be sure to type it in the body of the email and not the subject.


Building Access Registration

To obtain access to both the SPH-West and SPH-PI buildings after regular hours you must sign-up on a list that will be available from your division’s academic coordinator and also available in room 181 SPHPI. Add your signature next to your name and UIC ID number (Be sure that ALL numbers from your UIC I-Card including the black and blue numbers are included on the list next to your name; on new cards they are the bottom numbers.)
