Transfer service to individually liable


Individuals that wish to transfer their service to a "personal accounts" will be able to do so at events being hosted here on campus. You will be given two options for the transfer:

  1. Transfer the number only, return the equipment to ACCC. In order to transfer the phone number to your personal account please download and complete the form below labeled "Personally Liable Form". Once you have completed the form return your equipment with your name and information somehow affixed to the phone to your departmental business manager after attending one of the events on campus where you will be able to transfer your number over to a new plan. Please note that at the events on campus the vendors will be offering equipment upgrades that may allow for new equipment and contracts at little or no cost.

  2. Transfer the number, purchase the equipment. In order to transfer the phone number to your personal account and purchase the equipment please download and complete the form labeled "Personally Liable Form" as well as the "Mobile Device Purchase Form". Once both forms have been completed please contact the ACCC by sending a note to cellphones@helpdesk.cc.uic.edu; you will receive a response about the actual cost of the equipment. The cost of the phone to the employee will be calculated using a straight-line depreciation over four years. For example, if the original cost of the phone was $100, and the phone is one year old, the cost to the employee is $75. If it is two years old, the cost is $50, etc.. Send the completed forms along with a check for the confirmed purchase price to:

Events to facilitate the transferring of numbers to personal accounts will be hosted here on campus during the weeks of 12/5/11 and 12/12/11. More details will be posted soon.
